摘  要:目的:探讨血清及尿中Kim-1及CysC联合监测在临床诊断急性肾衰竭中的意义及价值。方法:选取我院重症监护病房的泌尿系统疾病患者108例根据急性肾衰竭诊断标准将其分为两组:58例符合急性肾衰竭诊断标准为ARF组,50例非急性肾衰竭为非ARF组,另取正常体检的健康人5 0例为正常对照组,检测三组患者的血清肌酐浓度、血清尿素氮、血清和尿KIM-1及Cys-C浓度,进而对血清和尿KIM-1及Cys-C水平进行分析。结果:与对照组和非ARF组相比,ARF组血清肌酐浓度、血清尿素氮、血清和尿KIM-1及Cys-C水平较高(P<0.05)。与血清和尿KIM-1及Cys-C水平的单独检测相比,两者联合检测敏感性以及特异性较高(P<0.05)。结论:血清及尿中Kim-1及CysC水平升高可作为急性肾衰竭的评估指标,两者联合监测对急性肾衰竭患者的诊断具有重要意义,对疾病预后有重要指导价值。Objective: To investigate the significance of combined detection of Kim-1 and CysC in plasma and urine in the clinical diagnosis of acute renal failure. Methods: 108 patients from our hospital to Intensive Care Unit of urinary system diseases were selected and divided into two groups according with diagnostic criteria, in which 58 cases with acute renal failure were selected into the ARF group, and 50 cases with non acute renal failure were the non ARF group, and another 50 healthy people were selected as the normal control group. The serum creatinine concentration, blood urine nitrogen, serum and urine KIM-1 and Cys-C levels were detected and compared. Results: Compared with the non ARF group and the normal control group, the serum creatinine concentration, blood urine nitrogen, serum and urine KIM-1 and Cys-C levels were higher(P〈0.05).Compared with the single detection of KIM-1 and Cys-C levels of serum and urine, the sensitivity and specificity of combined detection were higher (P 〈0.05). Conclusions: The plasma and urine KIM-1 and CysC levels are increased as the evaluation indexes of acute renal failure, and the combined detection has important value in the diagnosis and prognosis of acute renal failure.
