Miller金字塔示教法在骨科进修医师肩关节体格检查教学中的效果研究

摘　　要：目的：探讨Miller金字塔示教法在骨科进修医师肩关节体格检查教学中的效果，以期寻求更为有效的带教方法。方法以2011至2013年在我科学习的96名进修医师作为研究对象，随机分成A、B两组，每组48人。 A组采用传统示教法，B组采用注重理论与实践相结合的Miller金字塔示教法，分别进行肩关节体格检查教学。从专业基础知识、操作技能和临床诊断应用方面对两组进修医师进行考核，采用SPSS 19.0对两组考核结果行t检验。结果在专业基础知识测试方面，B组学员平均得分优于A组[（92.8±5.0） vs.（83.0±8.5）]；在操作技能考核方面，B组学员平均得分优于A组[（94.0±4.1） vs.（79.3±3.9）]；在临床诊断应用方面，B组学员平均得分优于A组[（90.6±2.3） vs.（85.1±1.9）]。各项考核指标两组间差异均有统计学意义（P〈0.05）。结论 Miller金字塔示教法在进修医师肩关节查体培训中的应用效果明显优于常规带教方法。Objective To explore the effect of Miller pyramid demonstration in the teaching of shoulder physical examination for visiting orthopaedic surgeons and aim at finding a more effective teach-ing method in this area. Method 96 visiting surgeons in our department were chosen and divided even into two groups randomly from 2011 to 2013. Traditional teaching method was applied in group A, while Miller pyramid demonstration which combined theory with practice was used in group B. Basic knowledge, manipulative skill and clinical diagnosis were assessed in group A and group B respectively. Finally, the results were statistical analyzed using t-test with SPSS 19.0 software. Results In the basic professional knowledge test, the average score of Group B was better than Group A [（92.8 ±5.0） vs. （83.0 ±8.5）]; in terms of operational skills assessment, the average score of Group B was better than Group A [（94.0 ±4.1） vs. （79.3± 3.9）]; in the clinical diagnosis of the application, the average score of Group B was better than Group A [（90.6±2.3） vs. （85.1±1.9）]. There were significant differences between the two groups （P〈0.05）. Conclusion Miller pyramid demonstration was more effective than traditional teaching method in the teaching of shoulder physical examination for visiting orthopaedic surgeons.
