CT三维成像技术指导微创手术对慢性泪囊炎患者血清血管内皮生长因子与降钙素原的影响

摘　　要：目的探讨CT三维成像技术指导微创手术对慢性泪囊炎患者血清血管内皮生长因子(VEGF)与降钙素原(PCT)的影响。方法选取该院慢性泪囊炎患者184例(276眼),采用随机数字表法将患者分为观察组(92例,139眼)与对照组(92例,137眼),对照组采用鼻泪管逆行硅胶管植入术,观察组采用CT三维成像技术指导微创手术,采用酶联免疫吸附法(ELISA)测定血清VEGF水平,采用酶联荧光分析法(ELFA)检测血清PCT水平。结果观察组完全治愈率与总治愈率明显高于对照组,无效率明显低于对照组(P<0.05)。观察组发热率、手术切口感染率、肺炎链球菌检出率与表皮葡萄球菌检出率均明显低于对照组(P<0.05)。治疗前,两组血清VEGF与PCT水平比较无统计学差异(P>0.05),治疗后,两组上述指标均明显低于治疗前,且观察组明显低于对照组(P<0.05)。结论 CT三维成像技术指导微创手术有助于显著改善慢性泪囊炎患者的临床疗效,可能通过降低血清VEGF与PCT水平,从而降低感染发生风险。Objective To investigate the CT imaging technology to guide minimally invasive surgery on serum vascular endothelial growth factor（ VEGF） and original procalcitonin（ PCT） in patients with chronic dacryocystitis. Methods 184 patients with chronic dacryocystitis（ 276 eyes） were seleted and,patients were divided into observation group（ 92 cases,139 eyes） and the control group（ 92 cases,137 eyes） by using a random number table. Patients in the control group were treated with nose lacrimal silicone tube implantation retrograde,while patients in observation group were used CT imaging technology to guide minimally invasive surgery. Serum VEGF level was detected by ELISA and PCT by ELFA. Results The rate of complete cure and the cure were significantly higher in observation group than those of control group,rate of inefficiency in observation group was lower than that of control group,there were all significant differences（ P〈0. 05）. Rates of fever,surgical wound infection,Streptococcus pneumoniae and Staphylococcus epidermidis detection rate of observation group were significantly lower than those of control group,the differences were statistically significant（ P〈0. 05）. Before treatment,there were no significant differences in the levels of serum VEGF and PCT（ P〈0. 05）,after treatment,above indexes of all patients were significantly lower than before treatment,above indexes of observation group were significantly lower than those of control group,the differences were statistically significant（ P〈0. 05）. Conclusions CT three-dimensional imaging techniques to guide minimally invasive surgery helps to significantly improve the clinical efficacy in patients with chronic dacryocystitis,possibly by reducing serum levels of VEGF and PCT,thereby reducing the risk of infection.
