中医内外合治小儿咳嗽变异性哮喘的疗效评价

摘　　要：目的:观察中药直流电离子导入联合益肺养阴胶囊治疗小儿咳嗽变异性哮喘(CVA)的中医临床疗效。方法:CVA患儿共93例,对照组31例予辅舒酮吸入及支气管扩张剂治疗,治疗1组30例在以上对照组治疗基础上,首周加予中药直流电离子导入;其后则单纯以口服益肺养阴胶囊治疗。治疗2组32例在对照组治疗基础上,首周予加中药直流电离子导入治疗;其后在对照组缓解期治疗方案的基础上加服益肺养阴胶囊口服。总疗程3月。结果:治疗首周后,中药直流电离子导入组(治疗1组与治疗2组)在咳嗽单症状评分及症状总评分方面均优于对照组(P<0.05),而咽痒及痰量症状评分则无显著性差异。治疗结束后,治疗2组在咳嗽、咽痒及总症状评分方面均优于对照组(P<0.05)。对照组在咳嗽单症状及症状总评分上均显著优于治疗1组(P<0.01)。两治疗组比较,治疗2组在咳嗽、咽痒及总症状评分方面均优于治疗1组(P<0.01)。比较中医证候疗效时发现,对照组总有效率为80.6%,治疗1组为76.7%,治疗2组为93.6%,治疗2组要高于其余两组,但尚未构成统计学差异;治疗1组与对照组两者比较亦无显著性差异;控显率方面,对照组、治疗1组及治疗2组分别为61.3%、60.0%及84.4%,治疗2组与其余两组差异显著(P<0.05),治疗1组与对照组则无显著性差异。同时,治疗后的三组患者血清IgE及FeNO与治疗前比较差异显著(P<0.01);治疗2组治疗后FeNO优于治疗1组及对照组(P<0.05);对照组的IgE、FeNO较治疗1组下降更显著(P<0.01)。所有患者在接受治疗方案规定的治疗剂量和疗程中未出现严重不良反应。结论:在西医常规治疗基础上加用中药直流电离子导入能更迅速有效地缓解CVA患儿的症状。益肺养阴胶囊治疗小儿CVA的疗效确切,当与西医常规联用时具有更高的控显率。Objective： To observe the clinical effect of direct current electric ion of Chinese traditional medicine combined with Yifeiyangyin capsule for the treatment of children＇s cough variant asthma（ CVA）. Methods： To observe 93 patients of CVA who accepted direct current electric ion of Chinese traditional medicine combined with Yifeiyangyin capsule treament. All of these patients were form the outpatient department of pediatrics and pulmonary of Guangdong Province Hospital of Integrated Chinese and Western Medicine. Among them,31 cases in the control group were treated with Fluticasone Propionate Inhaled Aerosol. 30 cases in the group one of treatment were treated with the basis of inhalation therapy combined with direct current electric ion of Chinese traditional medicine in the first week,and then treated with Yifeiyangyin capsule alone. 32 cases in the group two of treatment were also treated with the inhalation therapy combined with direct current electric ion of Chinese traditional medicine in the first week,and then recived the inhalation therapy and Yifeiyangyin capsule treatment which lasted to the end. The period of treatment was 3 months. Result： After the first week of treatment,the cough symptom score of direct current electric ion of Chinese traditional medicine groud（ group 1 and group 2） were better than the control group（ P〈0. 05）. But there is no significant difference on the improvement of throat itching and sputum volume. After 3 months of treatment,the cough symptom score and throat itching symptom score ofgroup 2 were significantly improved compared to the control group and group 1（ P〈0. 05）. In the same time,the cough symptom score and throat itching symptom score of control group is lower than the group 1（ P〈0. 05）. The total effective rate of the control group was 80. 6%,while the group 1 and group 2 were 76. 7% and 93. 6%. There is no significant different between them. For the control rate,the control group was 61. 3%,while the group 1 and group 2 were 60. 0
