血必净注射液与血液灌流联用对急性百草枯中毒的实效性评价

摘　　要：目的:观察并探讨血必净注射液与血液灌流(hemoperfusion HP)联用治疗急性百草枯(Paraquat,PQ)中毒患者的临床效果。方法:选择自2011年4月—2015年5月在医院接受治疗的急性PQ中毒患者52例,在常规PQ中毒急救处理的基础上分为灌流组(对照组26例)和灌流联合血必净治疗组(观察组26例)。统计并比较两组治疗前后动脉血气指标(PaO_2和PaCO_2)、脏器损伤血清学指标,包括谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平和肌酐(Cr)及预后情况。结果:对照组死亡16例,治愈10例,治愈率38.4%;观察组死亡8例,治愈18例,治愈率69.2%,观察组死亡率显著低于对照组,两组相比差异具有统计学意义(P<0.05);两组患者治疗后ALT、AST、CK、CK-MB和Cr明显下降,观察组显著低于对照组,差异具有统计学意义(P<0.05)。结论:血必净注射液与HP联用疗法可以保护PQ中毒患者的内脏正常功能,减少多脏器功能障碍发生,有效提高治愈率。Objective： To observe and explore the effect of Xuebijing Injection combined with hemoperfusion（ HP） in the treatment of acute paraquat（ PQ） poisoning. Methods： 52 acute PQ poisoning patients accepted treatment from April2013 to May 2015 in this hospital were selected and divided into two groups according to the random number method,26 cases in each group. Based on PQ poisoning first aid treatment,the patients were divided into hemoperfusion group（ control group,26 cases） and perfusion combined with Xuebijing Injection treatment group（ observation group,26 cases）. We comprehensively observed serum ALT,AST,CK,CK- MB and Cr of patients. Results： In control group,16 cases died and10 cases were cured,while 8 cases died and 18 cases cured in observation group. The mortality of the observation group was significantly lower than that of the control group and the difference between the two groups was statistically significant（ P 0. 05）. In two groups,especially in combined treatment group,serum ALT,AST,CK,CKMB,LDH and Cr levels statistically decreased and the difference was statistically significant（ P 0. 05）. Conclusion： Xuebijing Injection combined with hemoperfusion can protect normal functions of major organs and raise cure rate and also can help to reduce the possibility of functional impairment.
