鼻咽癌放疗患者自我感受负担及其影响因素的研究

摘　　要：目的调查鼻咽癌放疗患者自我感受负担水平,并分析其影响因素。方法采用一般资料调查表、自我感受负担量表、身体功能状态量表、心理痛苦体温计和Zarit护理负担量表对117例鼻咽癌放疗患者进行问卷调查。结果鼻咽癌放疗患者自我感受负担得分为(33.18±4.54)分,呈中度水平。多元回归分析显示放射性口腔黏膜反应程度、身体功能状态、照顾者负担水平、患者心理痛苦水平、患者年龄是其自我感受负担的主要影响因素(P<0.05)。结论护理人员应重视患者和照顾者自我感受负担水平,给予个性化的护理降低鼻咽癌放疗患者自我感受负担。Objective To investigate the level and influencing factors of self-perceived burden in patients with nasopharyngeal carcinoma undergoing radiotherapy. Methods Totally 117 patients with nasopharyngeal carcinoma undergoing radiotherapy and 117 caregivers were investigated with general information questionnaire,Self-Perceived Burden Scale（SPBS）,Karnofsky Performance Scale（KPS）,Distress Thermometer（DT） and Zarit Caregiver Burden In- terview（ZBI）. Results The self-perceived burden score of nasopharyngeal carcinoma patients undergoing radiother- apy was in moderate level. The main influencing factors included the degree of radiotherapy-induced oropharygeal mucositis,KPS,earegivers＇ burden,patients＇ psychological distress level and patients＇ age（P〈0.05）. Conclusion Nursing staff should pay attention to the self-perceived burden of patients and earegivers,and personalized nursing should be provided to reduce the self-perceived burden of patients with nasopharyngeal carcinoma undergoing radiotherapy.
