摘 要:目的应用超声造影研究颈动脉粥样硬化斑块内新生血管密度与血浆脂联素(APN)浓度的关系。方法颈动脉粥样硬化伴斑块形成患者67例,按脑梗死及斑块超声造影增强情况将其分为梗死组45例和非梗死组22例,采用免疫散射比浊法测定血浆APN浓度,比较两组血浆APN水平。结果梗死组血浆APN浓度(5.62±1.79)mg/L低于非梗死组(10.45±3.26)mg/L,差异有统计学意义(P<0.01),斑块增强0~Ⅱ级的血浆APN浓度分别为(12.75±3.83)mg/L、(7.32±2.54)mg/L及(2.63±1.24)mg/L,各组间两两比较差异均有统计学意义(P<0.01)。结论通过检测血浆APN水平,能间接评估颈动脉粥样硬化斑块易损性。Objective To explore the correlation between the density of neovascularization in carotid arteriosclerosis plaque and the concentration of adiponectin( APN),by using contrast- enhanced ultrasound.Methods Sixty- seven patients with carotid arteriosclerosis plaques were confirmed by ultrasound and contrast-enhanced ultrasonography. According to the contrast intensity in the plaque and the cerebral infarction occurrence,patients were divided into two groups: the cerebral infarction group( n = 45) and non- cerebral infarction group( n = 22). Serum APN concentration was determined by enhanced immune- turbidimetic assay.Results The Serum APN concentration in cerebral infarction group [( 5. 62 士 1. 79) mg / L] was significantly lower than that of the non- cerebral infarction group [( 10. 45 士 3. 26) mg / L],there was significant difference( P〈 0. 01). While significant difference was found among three types of plaques( P〈0. 01) : type 0 [( 12. 75士 3. 83) mg/L],type I [( 7. 32 士 2. 54) mg/L] and type II[( 2. 63 士 1. 24) mg/L]. Conclusions The concentration of serum APN is feasible to evaluate the vulnerability of the carotid arteriosclerosis plaques.
