摘 要:目的:探讨对老年苯二氮类(BZDs)药物依赖病人采用曲唑酮替代治疗的临床疗效及对病人认知功能的影响。方法:对四川省宜宾市第二人民医院2012年9月至2013年9月收治的62例BZDs药物依赖病人采用前瞻性研究,对其进行剂量递减和曲唑酮替代疗法。观察替代治疗前与6个月后病人的抑郁、睡眠状况、认知功能变化及药品不良反应(ADRs)发生情况。结果:替代治疗前与替代治疗后匹兹堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)中入睡时间、睡眠时间、睡眠效率比较差异无统计学意义(P>0.05);替代治疗后在睡眠质量、睡眠障碍、日间功能障碍和总分的评分[(1.96±0.43)、(1.85±0.33)、(2.22±0.46)和(12.81±1.15)]显著低于替代治疗前[(2.31±0.41)、(2.42±0.39)、(2.64±0.35)和(14.18±1.08)](P<0.05)。替代治疗后62例病人的韦氏量表的各项得分均显著高于替代治疗前(P<0.05)。结论:对BZDs依赖性病人采用曲唑酮替代治疗能够取得较好的治疗效果,同时改善病人的认知功能。曲唑酮能够很好地替代BZDs药物进行治疗。Objective: To explore the clinical efficacy of trazodone alternative therapy (AT) on benzodiazepines (BZDs)- dependent elderly patients and the effect on cognition of the patients involved. Methods: A prospective research was made on 62 BZDs-dependent elderly patients admitted into the Sichuan Yibin Second People's Hospital from Sep 2012 to Sep 2013. The patients received treatment with decreased dosage and trazodone AT. The depression, sleep status, changes of cognitive function in the patients and incidence of adverse drug reactions (ADRs) were observed before AT and 6 months after AT. Results: There were no significant differences in fall-asleep time, sleep duration and sleep efficiency of the patients measured by the scales of Pittsburgh Sleep Quality Index (PSQI), when comparisons were made between before AT and after AT (P〉0.05). Following AT, the scores of sleep quality, sleep disorder and the disorder of daily activities, as well as total scores [(1. 96± 0.43), (1.85±0.33), (2.22±0.46) and (12. 81±1.15)] were significantly lower than those [-(2. 31±0.41), (2.42± 0.39), (2.64±0.35) and (14.18±1.08)1 before AT (P〈0.05). After AT, the scores of all the indicators in Wechsler Scale of the 62 patients were significantly higher than those before AT (P〈0. 05). Conclusion: The AT could produce relatively good therapeutic effect on BZDs-dependent patients, and at the same time it could improve the cognitive function of the patients. For this reason, trazodone could replace BZDs in the treatment of the disease.
