成都市民营医疗机构手卫生现状调查与对策探讨

摘　　要：目的调查成都市目前民营医疗机构手卫生现状,探讨如何提高民营医疗机构手卫生对策。方法采用问卷调查、现场查看的形式,对成都市不同圈层分布的民营医疗机构手卫生现状进行调查。结果调查的民营医疗机构中诊所占71.6%。洗手设施以手触式水龙头为主(66.4%)。24.2%诊所未配备水龙头。有27.0%的诊所、23.8%的门诊部和53.3%的医院未配备干手设施。洗手液均以固体肥皂为主(64.0%)。采取肥皂干式保存的比例为诊所49.4%、门诊部58.8%和医院33.3%。有66.7%门诊部和61.8%的医院医务人员手卫生依从性较好。诊所和门诊部分别有35.4%和75%未参加过院外手卫生知识培训,有75%的门诊部和35.4%诊所医务人员未参加过手卫生知识培训。医院则参加过国家级、省级和市级各类培训。结论此次调查的民营医疗机构手卫生现状存在诸多薄弱环节,手卫生设施不完善,医务人员手卫生依从性不高,手卫生知识欠缺,应通过改善基础设施,加大医务人员尤其是诊所的院感知识培训力度,强化规章制度执行力,加强监管等手段进一步提高成都市民营医疗机构医院感染管理水平。Objective To investigate the current status of hand hygiene in Chengdu private medical institutions, discuss the countermeasure to improve hand hygiene. Methods The hand hygiene status was investigated in pri- vate medical institutions by questionnaire and field survey in different areas of Chengdu. Results There 71.6% medical institutions were clinics. The hand -touch faucets were the main hand washing facilities. There were 24.2% clinics no faucet; there were 27% clinics, 23.8% outpatient departments and 53.3% hospitals without hand dryer. Soaps （64. 0% ） were mainly used in private medical institutions. The dry stored soaps were 49.4% in clinics, 58.8% in outpatient departments and 33.3% in hospitals. The compliance of hand hygiene was satisfied in 66. 7% clinics and 61.8% hospitals. There were 35.4% clinics and 75.0% outpatient depart- ments had no hand hygiene training, and the hospitals had regular hand hygiene training. Conclusion In this survey, there were several parts of hand hygiene should be strengthened, including the facilities of washing hand, compliance of medical staff, education of hand hygiene, application of regulation and supervision, to im- prove the nosocomial infection management in private medical institutions.
