摘 要:目的观察分析细菌感染所致慢性阻塞性肺疾病(简称慢阻肺)患者血清CD64感染指数及PCT水平在早期诊断的价值及治疗前后的变化。方法选取慢阻肺患者850例,根据慢阻肺分期不同分为慢阻肺急性加重的A组634例,慢阻肺稳定期的B组216例,选取同期健康体检者100例为对照组C组,比较三组患者血清CD64感染指数及PCT水平差异及慢阻肺急性加重患者中细菌感然者治疗前后变化情况。结果A组患者CD64感染指数及PCT水平显著高于B组及C组患者,A组中细菌感染组患者CD64感染指数及PCT水平显著高于非细菌感染组患者及C组,A组中细菌感染组患者治疗后CD64感染指数及PCT水平均出现显著性下降,但治疗后CD64感染指数仍高于正常健康体检的C组,差异均有统计学意义(P<0.05)。结论CD64感染指数及PCT是细菌感染慢阻肺患者早期识别的敏感指标,且两者在细菌感染慢阻肺患者抗生素治疗前后变化明显,及早对慢阻肺急性加重患者进行CD64感染指数及PCT水平测定,能够较早的对细菌感染慢阻肺进行鉴别,在细菌感染慢阻肺患者治疗中及时监测能够更好的指导合理的抗生素应用。Objective To observe and analyze the serum CD64 infection index and PCT level changes of COPD caused by bacterial infection.Methods 850 COPD patients were divided into two groups according to the dif-ferent stages of COPD,and another 100 healthy people were taken as the control group.Their CD64 infection index and PCT level and bacteria infections were observed.Results Their CD64 infection index and PCT level was signifi-cantly higher in the group A than in the group B and the group C,and they were significantly higher patients with bacterial infection than in patients with non-bacterial infection and the group C,but the CD64 infection index was still higher in patients with bacterial infection of the group A than in the control group (P 〈0.05).Conclusion CD64 infection index and PCT are sensitive indicators for early identification of COPD patients with bacterial infection ,and they changes obviously before and after antibiotic treatment in COPD patients with bacterial infection.The early moni-toring of CD64 infection index and PCT level in AECOPD patients can better guide the rational use of antibiotics.
