摘  要:目的观察环孢素A对大鼠重症急性胰腺炎早期损伤和炎症反应的影响。方法将40只SD大鼠随机分为对照组(n=8)、重症急性胰腺炎组(n=16)和环孢素A组(n=16)。采用3.3%牛磺胆酸钠胆管注射法制备大鼠重症急性胰腺炎模型,环孢素A组在重症急性胰腺炎制模成功后40min再经腹腔注射环孢素A(50mg/kg)。重症急性胰腺炎组和环孢素A组大鼠各分为6h和24h两个亚组,对照组大鼠于24h时处死。抽取下腔静脉血测定血淀粉酶和C-反应蛋白,留取胰腺组织观察组织形态学变化并进行病理评分,通过免疫组化检测胰腺组织NF-κB p65的表达。结果重症胰腺炎组血淀粉酶、C反应蛋白、胰腺病理评分和NF-κB p65的表达均较对照组升高,并随时间延长而进一步升高(P<0.01);同时间段环孢素A组与重症胰腺炎组相比,血淀粉酶、C反应蛋白、胰腺病理评分和NF-κBp65的表达均降低(P<0.01)。NF-κBp65的表达与胰腺病理评分正相关(r=0.982,P<0.01)。结论环孢素A可明显减轻大鼠重症急性胰腺炎早期胰腺损伤和炎症反应程度,其机制可能与抑制NF-κB的表达有关。Objective To observe the effect of cyclosporine A on injury and inflammation in rats with early severe acute pancreatitis. Methods 40 rats were divided randomly into the control group (n = 8), severe acute pancreatitis (SAP) group (n=16) and Cyclosporine A (CsA) group (n=16). The rat of SAP group was induced by bile duct injection with 3.3% taurocholic acid. In the CsA group, 50 mg/kg of CsA was injected intraperitonealy 40 minutes after SAP was induced. SAP group and CsA group were divided into 6 h and 24 h subgroup respectively, and the rat of control group was sacrificed 24 hours after operation. Serum amylase and C- reactive protein were measured by extracting the in- ferior vena venous blood, morphological changes and pathological score of pancreatic were observed, and the expression of NF-kB p65 in pancreatic tissue was detected by immunohistochemistry. Results Serum amylase, C- reactive protein, pathological score and expression of NF-kB p65 in SAP group increased compared with the control group, and further in- creased with time(P〈0. 01). Compared with that of the SAP group on the same time point, serum amylase, C- reactive protein, pathological score and expression of NF-xB p65 decreased significantly(P〈0. 01). The expression of NF-kB p65 was positively correlated to pancreatic pathological score(r=0. 982, P%0.01). Conclusion Cyclosporine A can relieve the pancreatic injury and inflammation in rats with early severe acute panereatitis, and the mechanism may be relate to in- hibiting the expression of NF-kB.
