摘 要:目的探讨不同血液灌流吸附材料对急性百草枯中毒患者血清IL-6、IL-8和氧自由基损伤的影响。方法40例急性百草枯中毒患者随机分入A组和B组各20例。A组采用HA-230树脂血液灌流,B组采用HA-330树脂血液灌流。两组均在内科综合治疗基础上分别在中毒12小时内进行2小时血液灌流治疗。血液净化治疗前、后取静脉血检测IL-6、IL-8、MDA和SOD的含量。结果 B组治疗后血清IL-6下降(9.12±4.98)ng/L,A组下降(6.29±2.17)ng/L,B组IL-6下降较A组明显,差异有统计学意义(P<0.05)。B组治疗后血清IL-8下降(15.77±17.61)ng/L,A组下降(7.00±2.49)ng/L,B组IL-8下降较A组明显,差异有统计学意义(P<0.05)。B组治疗后血清MDA下降(1.97±1.07)ng/L,A组下降(1.34±0.62)ng/L,B组MDA下降较A组明显,差异有统计学意义(P<0.05)。B组治疗后血清SOD上升(22.44±10.63)ng/L,A组治疗后血清SOD上升(14.13±5.28)ng/L,B组SOD上升较A组明显,差异有统计学意义(P<0.05)。结论急性百草枯中毒患者采用的HA-330血液灌流器进行灌流后对降低血清中IL-6、IL-8浓度和减轻氧自由基损伤效果明显优于HA-230血液灌流器。可作为抢救急性百草枯中毒患者的临床选择。Objective To investigate different adsorbing materials of hemoperfusion on the effects of IL-6,IL-8and damage induced by oxygen free radical in serum of patients with acute paraquat poisoning.Methods 40 patients with acute paraquat poisoning were randomly assigned to group A and group B,and each group contained 20 cases.HA-230 resin was used in the group A,and HA-330 resin was used in the group B.All patients were given the conventional treatment and hemoperfusion for two hours after poisoning in 12 hours.IL-6,IL8,SOD and MDA in serum before and after the hemoperfusion were measured.Results Compared with those in the group A,the levels of IL-6,IL-8and MDA in the group B were obviously decreased after the hemoperfusion(P0.05).The activity of plasma SOD in the group B was increased more than that in the group A(P0.05).Conclusion Compared with HA-230 type hemoperfusion,HA-330 type hemoperfusion not only can effectively reduce the patients serum IL-6and IL-8,but also decrease the damage induced by the oxygen free radical.
