同时与序贯无痛胃肠镜的应用效果评价

摘　　要：目的研究同时与序贯无痛胃肠镜两种检查方式的应用效果,并对其安全性进行评价。方法选取在消化内镜室行无痛内镜检查的54例患者作为研究对象,采用双盲法随机分为同时无痛胃肠镜检查组(研究组)和序贯无痛胃镜检查组(对照组),每组各27例。比较两组患者的检查时间、丙泊酚用量、血氧饱和度(SpO2)等相关指标,并对安全性进行评价。结果研究组检查用时、苏醒时间、丙泊酚用量及单元操作间检查数与对照组相比,均具有统计学差异(P<0.05)。术后比较两组患者的SpO2、收缩压(SBP)、舒张压(DBP)及心率(HR)发现,无显著性差异(P>0.05)。研究组不良反应发生率7.4%,满意率100%,对照组不良反应率22.2%,满意率88.9%,研究组优于对照组,有统计学差异(P<0.05)。同时无痛组在安全性方面要优于序贯无痛组,在对患者的血压、心率、血氧饱和度变化观察过程中,同时无痛胃肠镜与序贯无痛胃肠镜相比血压、心率异常明显要少,血氧保护度变化明显增多。结论同时无痛胃肠镜和序贯无痛胃肠镜均能完成内镜检查。但前者检查用时、苏醒时间及满意率优于后者,且不良反应低,安全性高,患者易接受,临床值得推广运用。Objective To study the effect and safety of simultaneous painless and sequential painless gastrointestinal endoscopy.Methods From January 2015 to February 2015,54 patients with endoscopy in our hospital were selected as the research object.The patients were randomly divided into painless gastroscopy group（study group,27cases）and painless gastroscopy group（control group,27cases）.The examination time,propofol dosage,SpO2 and other related indicators were observed.Results The test time,recovery time,propofol dosage and unit operation of the study group were significantly different from that of control group（P0.05）.The SpO2,DBP,SBP and HR of the two groups were found to have no significant difference（P0.05）.The incidence of adverse reactions and satisfaction rate in the study group were 7.4% and 100%.The incidence of adverse reactions and satisfaction rate in control group were 22.2% and88.9%.The incidence of adverse reactions and satisfaction rate of study group were superior to those of the control group（P0.05）.The changes of blood pressure and heart rate of study group were significantly less than that of control group.Conclusion Painless gastrointestinal endoscopy can better complete endoscopic examination,reduce the pain of patients and adverse reaction is low and precept by the patients.
