自拟祛毒活络汤联合苯溴马隆对痛风急性期高血症患者尿酸排泄指标影响  

Effects of Self-designed Qudu Huoluo Decoction Combined with Benzbromarone on Uric Acid Excretion Indexes in Patients with Gout in Acute Stage and Hyperuricemia

摘　　要：目的:观察自拟祛毒活络汤联合苯溴马隆治疗痛风急性期高尿酸血症的临床效果。方法:纳入74例痛风急性期高尿酸血症患者,按随机数字表法分为对照组与治疗组,各37例,对照组单用苯溴马隆治疗,观察组加用自拟祛毒活络汤,监测两组治疗前后血尿酸(UA)、血肌酐(SCr)、24 h尿蛋白定量等指标的变化,观察治疗前后两组关节功能的变化情况,记录两组症状改善情况。结果:(1)治疗组总有效率为91.89%,明显高于对照组的62.16%,对比差异有统计学意义(P<0.05)。(2)治疗后,两组症状积分均明显降低,治疗组红肿、疼痛、发热、活动受限等积分降低幅度高于对照组(P<0.05)。(3)治疗后,两组尿酸排泄指标均有改善,治疗组UA、24 h尿蛋白定量、Scr均明显降低,幅度高于对照组(P<0.05)。(4)治疗后,两组关节压痛评分、肿胀个数及关节活动指数均有所改善,治疗组关节压痛评分降低显著、关节肿胀个数减少、关节活动指数改善,幅度均优于对照组(P<0.05)。结论:自拟祛毒活络汤联合苯溴马隆治疗痛风急性期高尿酸血症,疗效确切,患者关节功能改善明显,血尿酸指标改善显著。Objective ： To observe the clinical effects of Self-designed Qudu Huoluo Decoction combined with benzbromarone in the treatment of gout in acute stage and hyperuricemia. Methods ： 74 cases of patients with gout in acute stage and hyperuricemia were included in the study and were randomly divided into the control group and the treatment group according to the random number table, with 37 cases in each group. The control group were treated with benzbromarone alone while the observation group were additionally treated with Self-designed Qudu Huoluo Decoction. The changes of indexes such as blood uric acid （ UA ）, serum creatinine （SCr）,24 h urine protein quantitation in the 2 groups were monitored before and after treatment. The changes of joint function in the 2 groups before and after treatment were observed and the improvement of symptoms in the 2 groups were recorded. Results ： （ 1 ） The total effective rate in the treatment group （ 91.89% ） was significantly higher than that in the control group （ 62.16% ） （ P〈0.05 ）. （ 2 ） After treatment, the symptom scores of the 2 groups were significantly lower. The decreasing ranges of scores of red and swollen, pain, fever and limitation of motion in the treatment group were significantly higher than those in the control group （ P〈0.05 ）. （ 3 ） After treatment, the uric acid excretion indexes in the 2 groups were improved. UA, 24 h urine protein quantitation, Scr in the treatment group were significantly lower and the ranges were greater than those in the control group （ P〈0.05 ）. （ 4 ） After treatment, joint tenderness score, number of swelling and joint motion indexes in the two groups were improved. In the treatment group, joint tenderness score decreased significantly, number of joint swelling decreased and joint motion indexes were improved.
