摘 要:目的对两种人工髋关节置换治疗缺血性股骨头坏死(IFHN)的效果进行比较。方法选择本科收治的IFHN患者60例,采取骨水泥型假体进行人工髋关节置换术治疗29例(骨水泥组),采取生物型假体进行人工髋关节置换术治疗31例(非骨水泥组),分别对两组手术时间、术中出血量、切口感染、血管神经损伤以及假体松动、Harris评分进行比较,评价二者的治疗效果。结果两组术中失血量无统计学差异(P>0.05);骨水泥组手术时间明显长于非骨水泥组(P<0.05),骨水泥组仅1例发生切口感染,非骨水泥组仅1例发生假体松动,两组均未发生血管神经损伤事件。与术前相比,术后两组的Harris评分均显著增加(P<0.05);在术后1、6、12个月,两组Harris评分均无统计学差异(P>0.05)。结论对于IFHN,采用生物型假体或骨水泥假体进行人工髋关节置换术,均可以取得较好治疗效果,然而使用生物型假体进行人工髋关节置换术的手术时间较短。Objective To compare the effects of the replacement with two kinds of artificial hip joints on treatment of ischemic necrosis of femoral head (INFH). Metlaods A total of 60 IFHN patients admitted to the department were selected. Among which, 29 patients were treated with the replacement with bone cement hip joint (bone cement group) and 31 with biological hip joint (non-bone cement group). The duration of operation, amount of operative bleeding, incision infection, nervus vascularis damage and prosthesis loosening and Harris score of the two groups were compared in order to evaluate the effects of the two kinds of artificial hip joints. Results There was no significant difference between the amounts of operative bleeding in the two groups (P 〉 0.05); the duration of operation in the bone cement group was significantly longer than that in the non-bone cement group (P 〈 0.05); only one patient in the bone cement group suffered incision infection; only one patient in the non-bone cement group suffered prosthesis loosening; no nervus vascularis damage occurred in the two groups. The Harris score in the two groups was much higher than that before the operation (P 〈 0.05); there was no statistic difference between the Harris score of the two groups one, six and 12 months after the operation (P 〉 0.05). Conchmion The replacement with both bone cement and biological hip joints may achieve favorable effects on the treatment of IFHN. However, the duration of operation for the treatment with biological hip joint is shorter.
