微创手术和常规保守治疗踝关节退行性骨关节病临床疗效对比

摘　　要：目的分析微创手术和常规保守治疗踝关节退行性骨关节病患者临床功能恢复和疗效对比,为临床治疗提供参考。方法选取该院2013年8月—2015年2月收治踝关节退行性骨关节病患者240例为研究对象,随机分为微创组和对照组,对照组患者采取常规保守治疗,微创组患者采取微创手术治疗,比较患者临床功能恢复和疗效。结果患者均成功获得随访,均未出现感染情况,微创组(85.8%)和对照组疗效(82.5%)相当,差异无统计学意义(P>0.05)。治疗前,二组患者MaZur评分和Terner评分差异无统计学意义(P<0.05),经过治疗Mazur评分和Tegner评分显著提高,微创组Mazur评分(87.51±6.84)显著高于对照组(81.26±6.39)(P<0.05)。治疗前,VAS评分差异无统计学意义(P<0.05),经过治疗,二组VAS评分均显著降低,微创组疼痛VAS评分(2.43±0.51)显著低于对照组(3.08±0.16)(P<0.05)。结论微创手术和常规保守治疗踝关节退行性骨关节病具有很好效果,微创手术能够提高治疗效果,缓解疼痛。Objective To analyze the functional recovery and comparative clinical efficacy between minimally invasive surgery and conventional conservative treatment for ankle degenerative joint ,and to provide refer‐ence for clinical treatment .Methods Two hundred forty patients with ankle degenerative joint in hospital from August 2013 to February 2015 were selected ,the patients were randomly divided into minimally inva‐sive group and control group .The control group of patients was taken trentment of conventional conserva‐tive treatment ,while the minimally invasive group of patients was taken trentment of minimally invasive surgery .Clinical function recovery and efficacy of patients were compared .Results All the patients were successfully followed up ,no infection .The efficacy of the minimally invasive group （85 .8% ） and the con‐trol group （82 .5% ） was quite ,the difference was not statistically significant （P〉0 .05） .Before treatment , the MaZur scores and Terner score was no significant difference （P〈0 .05） between the two groups ,after treatment ,Mazur score and Tegner scores were significantly improved ,Mazur score of the minimally inva‐sive group （87 .51 ± 6 .84） was significantly higher than the control group （81 .26 ± 6 .39）（P〈0 .05） .Be‐fore treatment ,there was no significant difference in pain scores （P〈0 .05） between the the two groups , after treatment ,the pain scores were significantly lower in both groups ,it was significantly lower in the minimally invasive group （2 .43 ± 0 .51） than the control group （3 .08 ± 0 .16） （P〈 0 .05） .Conclusion Minimally invasive surgery and conventional conservative treatment of ankle degenerative bone joint has a good effect ,minimally invasive surgery can improve the therapeutic effect ,and relieve pain .
