改良式曼月乐与促性腺激素释放激素激动剂(GnRH-a)治疗大子宫腺肌病的临床疗效观察

摘　　要：目的 探讨对比应用改良式曼月乐与促性腺激素释放激素激动剂（Gn RH-a）治疗大子宫腺肌病的临床疗效。方法将广州开发区医院收治的大子宫腺肌病患者作为研究对象并实施分组研究,研究组采用改良式曼月乐治疗,对照组采用抑那通治疗,将两组患者的治疗效果、副反应及费用情况进行对比。结果 研究组子宫体积变化不明显,子宫内膜明显变薄,对照组治疗3～6个月时子宫体积明显缩小,但治疗12个月时子宫体积有所增大,子宫内膜增厚;两组痛经VAS评分均明显下降,对照组治疗1个月、3个月时月经量明显减少,但治疗12个月时有增多现象,仍少于治疗前（P〈0.05）;研究组治疗费用显著低于对照组（P〈0.05）;不良反应多于对照组（P〈0.05）。结论 两组相比,痛经均有所减轻,月经量均有所减少,治疗效果相当;远期效果研究组似优于对照组,治疗费用显著低于对照组,不良反应多于对照组。OBJECTIVE To explore the comparison of application of modified Mirena and gonadotropin releasing hormone agonist（GnRH-a） in the treatment of uterine adenomyosis clinical curative effect. METHODS The target hospital＇ s adenomyosis patients were selected as the object of study and implementation of packet of study group treated with modified Mirena, by inhibiting the treatment in the control group, the curative effect, side effect and cost were compared on two groups of patients. RESULTS The study group of uterine volume change is not obvious, endometrium was significantly thinner, the treatment group from 3 to 6 months uterine volume-was significantly reduced in control, but 12 months treatment uterine size increased, the endometrial thickened; dysmenorrhea VAS scores of the two groups were decreased significantly, the treatment group 1 and 3 months when menstrual volume decreased significantly in the controls, but 12 months treatment with increase in the phenomenon was still less than before treatment（P〈0.05）; cost of treatment in the study group was significantly lower than that of control group（P〈0.05）; adverse reactions than the control group（P〈0.05）. CONCLUSION Two groups compared to dysmenorrhea Were mitigated, menstrual quantity decreased, treatment effect, long-term effect of group appeared to be better than the control group. The cost of treatment was significantly lower than that of the control group, the adverse reaction was heavier than control group.
