摘 要:目的探讨持续颅内压检测在颅内动脉瘤、脑出血、重型脑损伤术后的临床应用价值。方法选取本院2014年8月~2015年2月于潍坊市人民医院神经外科进行开颅动脉瘤夹闭、颅内血肿清除术及脑室外引流术患者36例。采用Codman持续颅内压监护仪(美国强生公司生产),由主机和光电感应器组成。患者行开颅手术,放置感应器于硬脑膜下或脑室内,每次监测前均预先调零出现参考值。连续观察与记录ICP指数,同时严密观察意识瞳孔及生命体征变化。颅内压结果可作为随时调整治疗方案的重要参考,监测时间9~14 d,平均12 d。结果在颅内压监测下及时调整脱水药物及脑血管扩张药物的用量及频次,有效控制脑水肿及高颅压症状,绝大部分患者病情逐渐好转。结论使用颅内压监护可以明显提高颅内动脉瘤、脑出血、重型脑损伤患者的临床疗效.值得推广应用。Objective To investigate the clinical application value of continuous intracranial pressure monitoring for patients undergoing the surgery of intracranial aneurysm, cerebral hemorrhage or severe craniocerebral injury. Methods During August 2014 to February 2015, thirty-six patients from Weifang People’s Hospital undergoing craniotomy for intracranial aneurysm clipping, cerebral hemorrhage removring and external ventricular drainage were selected for analysis. Intracranial pressure data was continuously observed and recorded using Codman intracranial pressure monitor. Meanwhile, patients’ awareness, pupil and vital signs was observed strictly. The average monitoring time is 12 days. The therapy plan could be adjusted at any time according to the monitoring results. Results Through adjustment of dehydration and cerebrovascular expansion drugs under the continuous intracranial pressure monitoring, patients’ cerebral edema and high cranial pressure symptoms could be effectively controlled. Conclusion The usage of continuous intracranial pressure monitoring could obviously improve patients’ clinical curative effect included by intracranial aneurysm, cerebral hemorrhage and severe brain injury.
