丹蛭降糖胶囊对糖尿病合并冠心病患者血清CTRP-9及Hcy的影响

摘　　要：目的探讨丹蛭降糖胶囊对糖尿病合并冠心病患者血清肿瘤坏死因子相关蛋白-9(C1q/TNF-related protein-9,CTRP-9)及同型半胱氨酸(Hcy)的影响。方法选择住院糖尿病患者60例,其中2型糖尿病28例,简称糖尿病组;糖尿病合并冠心病32例,简称合并组,两组均在服用常规西药基础上加服丹蛭降糖胶囊(DJC)。另选取健康体检者30例,简称对照组。三组均行血生化检测,采用ELISA法检测患者血清CTRP-9水平。结果糖尿病组空腹血糖(FPG)、糖化血红蛋白(Hb A1c)、低密度脂蛋白胆固醇(LDL-C)、Hcy均高于对照组(P<0.05,P<0.01),高密度脂蛋白胆固醇(HDLC)、CTRP-9低于对照组(P<0.05);合并组Hcy水平高于糖尿病组,CTRP-9低于丹蛭组(P<0.05)。Pearson相关分析结果显示,Hcy与Hb A1c呈正相关(r=0.535,P=0.000),CTRP-9与Hb A1c呈负相关(r=-0.740,P=0.000);Hcy与CTRP-9呈负相关(r=-0.725,P=0.000)。结论 CTRP-9对糖尿病合并冠心病有保护作用,Hcy是糖尿病、冠心病患者的危险因子,丹蛭降糖胶囊可能有降低Hcy,提高CTRP-9的作用。Objective To observe the effects of Danzhijiangtang Capsule（ DJC） on the plasma C1 q / tumor necrosis factor- related protein- 9（ CTRP- 9） and homocysteine（ Hcy） in patients with diabetes mellitus with coronary heart disease. Methods 60 patients with diabetes mellitus were treated in Department of Endocrinology,first affiliated hospital of Anhui University of Traditional Chinese Medicine were divided into two group： Type 2 Diabetes Mellitus（ DM group for short,n = 28）; type 2 diabetes mellitus with coronary heart disease（ combined group for short,n = 32）,two groups were treated by routine method and DJC. The control group is composed of 30 healthy cases. The biochemical parameters and the levels of CTRP- 9 were detected by ELISA technique in three groups. Results The level of fasting blood glucose（ FBG）,glycosylated hemoglobin（ Hb A1c）,low density lipoprotein-cholesterol（ LDL- C） and Hcy were significantly higher,and the levels of high density lipoprotein- cholesterol（ HDL- C）,CTRP- 9 were lower in DM group than those in control group（ P〈 0. 01）. The levels of Hcy was higher,and the levels of CTRP- 9were lower in combined group than those in DM group（ P 〈0. 05）. Hcy was positively correlated with Hb A1c（ r = 0. 535,P =0. 000） and negatively correlated with CTRP- 9（ r =- 0. 725,P = 0. 000）. CTRP- 9 was negatively correlated with Hb A1c（ r =- 0. 740,P = 0. 000）. Conclusion CTRP- 9 have a protective effect on DM and CHD,Hcy was an independent risk factor of DM and CHD,DJC may have effect to reduce the level of Hcy and increase the CTRP- 9.
