消痛膏对肛裂疼痛及内括约肌痉挛影响的临床研究

摘　　要：目的验证消痛膏能否减少肛裂疼痛及有效地缓解了括约肌痉挛。方法随机抽取60名肛肠科二期(慢性)肛裂的患者按就诊先后顺序分成治疗组和对照组各30例。治疗组为消痛膏,对照组为0.2%硝酸甘油软膏,治疗组每天早中晚各用消痛膏涂抹溃口1次,对照组将0.2%硝酸甘油软膏早中晚涂抹溃口1次,疗程各7天,分别于用药前和用药后第3、5、7天观察其疼痛的程度,观察用药前和用药后便后肛门疼痛持续时间、用肛管直肠内压测量仪测量各组患者用药前和用药后30min第5天的"肛管静息压"。结果治疗组较对照组对肛裂内括约肌痉挛的患者疼痛程度减轻、便后肛门疼痛持续时间、治疗组的肛管静息压均比用药前有所下降,有显著性差异(P<0.05),治疗组优于对照组。结论消痛膏能使肛裂疼痛程度减轻、便后肛门疼痛持续时间缩短、缓解内括约肌痉挛、肛管静息压下降。Objective This clinical study is aimed to verify the efficacy of Xiaotong Ointment,which is applied to relieve the anal fissure pain and internal sphincter spasm,and promote healing. Methods In the experiment,60 outpatients of anal fissure sphincter spasm in 2nd chronic will be randomly selected as samples,and divided into 2 groups,30 for the treatment group and 30 for the control group. In the treatment group,Xiaotong Ointment will be used. In the control group,the 0. 2% Nitroglycerin Ointment would be used. The outpatients shall smear the ointment onto their wound 3 doses per day for 7 days in total. The pain degree,the duration of the after- excretion pain before and after ointment treatment will be observed on the 3rd,5th and 7th day in the experiment. And the anal canal resting pressure will be measured 30 mins before and 30 mins after the ointments treatment on the 5th day in the experiment. Results In the experiment for the outpatients of anal fissure sphincter spasm in 2nd chronic,in the treatment group,the duration of the after- excretion pain has been better relieved,the duration of the after- excretion pain has been shorten,and the anal canal resting pressure has been lower than that before the treatment,with the significant difference of P〈 0. 05,which is superior than those in the control group. Conclusion In conclusion,the Xiaotong Ointment can relieve the pain of anal fissure,shorten the duration of the after- excretion pain,ease the internal sphincter spasm,and lower the anal canal resting pressure.
