摘　　要：背景：股骨转子间骨折采用保守治疗或内固定治疗，一般效果不佳。目的：观察双极股骨头置换治疗老年转子间骨折的效果。方法：收集2012年1月至2014年6月收治的老年不稳定股骨转子间骨折患者62例，采取双极股骨头置换治疗。结果与结论：62例患者有55例获得有效随访，其中髋关节功能优32例，较好14例，良5例，差4例，优良率93％。提示双极股骨头置换手术操作时间短，卧床时间少、早期可负重活动，减少因卧床引起的术后并发症等优点，是治疗老年转子间骨折有效的方法。Background： Effect of conservative treatment or internal fixation for femoral intertrochanteric fractures is not well. Objective： To observe and approach the clinical therapeutic effectiveness of bipolar femoral head replacement in treatment of unstable femur intertrochanteric fracture in elder patients. Methods： A follow-up study was performed from January 2012 to June 2014. There were 62 patients with unstable femur intertrochanteric fractures. Bipolar femoral head replacement in treatment of unstable femur intertrochanteric fracture in elder patients. Results and Conclusion： Fifty five patients in this study were followed-up with telephone or out-patient interview. According to Haris hip score, the result showed excellent in 32 patients, better in 14 patients, good in 5 patients and poor in 4 cases, the rate of excellent and good was 93%. Application of bipolar femoral head replacement in treatment of unstable femur intertrochanteric fracture in elder patients has many advantages, which include less time occupied in operation and bed, early weight-bearing activities, and reducing the occurence of postoperative complications. And it is the effective treatment for the unstable femur intertrochanteric fracture in elder patients.
