摘　　要：应对老龄化社会,老年人长期照护问题是急需解决的重要问题。基于老年人的生理心理特点,老年人长期照护服务应包括健康和不健康的老年人,涉及生活需求、健康需求、心理需求和社会需求以及老年人临终关怀等特殊需求。社会工作在对老年人服务进行介入时,需要进行老年人长期照护需求的调查,并以此实际需求为基础,采用个案管理和规划、小组治疗和支持、社区照顾、临终关怀等专业模式和方法,满足老年人长期照护服务的需求。to deal with an aging society, the long-term care of the elderly is an important issue needed to be resolved. Based on the physiological and psychological characteristics of the elderly, the long -term care services for the elderly should include both the healthy and the unhealthy, involving life needs, health needs, psychological and social needs of older persons and the deathbed solicitude care and other special needs. When social work services intervenes with the elderly care services, investigation of the need for the long-term care services for the elderly is needed. Based on the actual needs, the adoption of case management and planning, group therapy, support and community care, deathbed solicitude care and other professional models and methods are needed to meet the needs of the long-term care of the elderly.
