摘  要:目的探讨血清学炎性感染指标降钙素原(PCT)、白细胞介素-6(IL-6)和中性粒细胞CD64指数在小儿肺部细菌性感染诊断中的临床价值。方法收集338例肺部感染患儿,经血培养、分泌物和肺泡灌洗液培养等病原学检查确诊细菌性感染192例,非细菌性感染146例,选取同期呼吸系统健康儿童60例作为对照组,三组均行PCT、IL-6和CD64指数检查。结果非细菌性感染组和对照组PCT、IL-6水平和CD64指数明显低于细菌性感染组(P<0.05)。PCT和CD64指数诊断小儿肺部细菌性感染的敏感性、特异性、阳性预测值及阴性预测值均高于IL-6(P<0.05)。结论 PCT和CD64指数诊断小儿肺部细菌性感染具有较好的敏感性及特异性,二者联合检测对细菌性感染的诊断具有重要意义。Objective To investigate the clinical value of procalcitonin (PCT),IL-6 and neutrophilic CD64 index in diagnosis of pulmonary bacterial infection in children .Methods In 338 children with pulmonary infection ,192 were diagnosed as bacterial infec-tion and 146 as non-bacterial infection through blood culture ,secretions and bronchoalveolar lavage fluid culture and other pathogenic examinations.At the same time,60 healthy subjects were used as control group .The levels of PCT,IL-6 and CD64 index were detected. The diagnostic value of PCT , IL-6 and CD64 index were analyzed and compared in children patients with pneumonia .Results Serum PCT,IL-6 and CD64 index in the bacterial infection group were much higher than that in the non-bacterial infection group and healthy control group (P〈0.05).The sensitivity,specificity,positive predictive value and negative predictive value of PCT and CD64 index were much higher than that of IL -6.Conclusion PCT and CD64 index have the higher sensitivity and specificity than IL-6 in diagnosis of bacterial infection .Combination of the two markers has an important significance for diagnosis of bacterial infection .
