脂微球前列地尔联合α硫辛酸治疗老年早期糖尿病肾脏疾病的临床观察

摘　　要：目的评价脂微球前列地尔联合α硫辛酸治疗老年早期糖尿病肾脏疾病（DKD）的疗效。方法选择2013年10月~2015年4月确诊的老年DKD患者220例,随机分为观察组和对照组,每组110例,两组均行常规降糖、降压等治疗,对照组采用微球前列地尔10μg加入生理盐水100 ml中静脉滴注,1次/d,观察组在此基础上联合α硫辛酸600 mg加入生理盐水100 ml中静脉滴注,1次/d,疗程均为3周。比较两组治疗前后血清肌酐（Cr）、血尿素氮（Bun）、血胱抑素（Cys C）、血β2微球蛋白（β2-MG）及尿微量白蛋白（m Alb）、尿视黄醇结合蛋白（U-RBP）、尿N-乙酰β-D氨基葡萄糖苷酶（NAG）水平。结果治疗3周后,两组m Alb、U-RBP、NAG水平均较治疗前明显下降,观察组下降更为显著,差异具有统计学意义（P〈0.05）;与对照组治疗后比较,观察组m Alb、U-RBP、NAG水平明显降低,差异具有统计学意义（P〈0.05）。结论脂微球前列地尔联合α硫辛酸治疗老年早期DKD有较好的临床疗效。Objective To evaluate the therapeutic effect of alprostadil combined with lipoicin for elderly patients with early diabetic kidney disease（DKD）. Methods Two hundred and twenty elderly patients with early diabetic kidney disease were divided into two groups： treatment group and control group. Treatment group was treated with alprostadil combined with α-lipoicin and control group was given only alprostadil. Cr,Bun,m Alb,Cys C,β2-MG,U-RBP and NAG of the two groups were compared before and after treatment. The course of treatment was three weeks. Results The m Alb,U-RBP and NAG levels of treatment group were significantly higher than those of the control group（P〈0.05）. Conclusion Alprostadil combined withα-lipoicin can improve the effect of treatment for elderly patients with early diabetic kidney disease.
