输尿管镜下尿道会师术治疗小儿尿道断裂9例临床效果分析

摘　　要：目的探讨输尿管镜下尿道会师术治疗小儿尿道断裂的临床效果。方法 18例尿道断裂患儿分为2组,行输尿管镜下尿道会师术治疗的9例患者为观察组,行开放手术尿道会师术9例患者为对照组。比较2组临床治疗效果和术后1a最大尿流率及尿道狭窄等并发症发生情况。结果观察组手术时间[(13.50±4.50)min]、住院时间[(8.12±2.50)d]短于对照组[(26.30±5.30)min、(16.50±3.50)d](P<0.05),术中出血量[(22.15±4.50)mL]少于对照组(84.50±6.50)mL;术后1a,观察组最大尿流率[(18.60±3.50)mL/s]与对照组[(19.30±2.90)mL/s]比较差异无统计学意义(P>0.05);观察组发生感染3例,尿道狭窄3例,阴茎勃起障碍3例,尿失禁1例;对照组发生感染2例,尿道狭窄2例,阴茎勃起障碍4例,尿失禁1例。结论输尿管镜下尿道会师术可达到与开放手术相同的临床治疗效果,创伤小,恢复时间短。Objective To investigate the therapeutic effect of ureteroscopic urethral reunion operation on children with urethral disruption. Methods Eighteen children with urethral disruption were divided into observation group receiving ureteroscopic urethral reunion operation, and control group receiving open urethral reunion operation, with 9 patients in each group. The clinical therapeutic effect, maximum flow rate and the incidence of urethral stricture one year after operation were compared between two groups. Results The operation lasting time （（13.50 ± 4.50） min） was shorter, the blood loss （（22.15±4.50） mL） was less, and the hospitalization stay （（8.12±2.50） d） was shorter in observation group than those in control group （（26.30±5.30） min, （84.50±6.50） mL, （16.50±3.50） d） （P〈0.05）. There was no significant difference in the maximum flow rate between observation group （（18.60±3.50） mL/s） and control group （ （19.30 ± 2.90） mL/s） （P〉0.05）. Urethral stricture, infection, erectile dysfunction and urinary incontinence occurred in 3, 3, 3 and 1 patient in observation group, and 2, 2, 4 and 1 patient in control group. Conclusion Ureteroscopic urethral reunion operation can achieve the same result as open urethral reunion operation, with small trauma and quick recovery.
