个体化护理对糖尿病肾病血液透析患者治疗依从性和心血管并发症的影响

摘　　要：目的探讨个体化护理对糖尿病肾病血液透析患者治疗依从性及心血管并发症的影响。方法糖尿病肾病行血液透析治疗患者50例随机分为实验组24例和对照组26例。对照组常规护理,实验组实施个体化护理干预。对比2组透析治疗依从性、临床疗效和并发症发生情况。结果实验组在控制体质量增加和配合医嘱选择透析方案方面的依从性显著优于对照组（P〈0.01）。干预后2组血清肌酐（Scr）、尿素氮（Bun）水平均较治疗前下降,且实验组改善程度显著优于对照组（P〈0.05或P〈0.01）。治疗的第7~12个月,实验组低血压、心力衰竭、心律失常发生率显著低于对照组（P〈0.05）。结论个体化护理干预能够显著提高糖尿病肾病血液透析患者的依从性,减少心血管并发症的发生率。Objective To investigate the effect of individualized nursing on treatment adherence and cardiovascular complication of diabetic nephropathy patients with continuous hemodialysis.Methods A total of 50 diabetic nephropathy patients undergoing continuous hemodialysis were randomly divided into the study group( n = 24) and the control group( n = 26). The control group was given conventional nursing care and the study group was received the individualized nursing intervention. The treatment adherence,clinical efficacy and incidence of cardiovascular complication were compared between two groups. Results The study group achieved better treatment adherence in body weight controlling and dialysis solution( P < 0. 01). The serum creatinine and urea nitrogen were decreased in both of two groups after treatment,and were significant lower in the study group than those of controls( P < 0. 05 or P < 0. 01). The study group had lower incidence of hypotension,cardiac failure and arrhythmia compared with the control group between seventh and twelfth month of treatment period( P <0. 05). Conclusion The individualized nursing can improve the treatment adherence and reduce the risk of cardiovascular complication in diabetic nephropathy patients with continuous hemodialysis.
