呋塞米持续静脉泵入联合静脉滴注高渗盐水治疗心力衰竭伴低钠血症临床观察

摘　　要：目的观察持续静脉泵入呋塞米及高渗盐水治疗心力衰竭伴低钠血症的临床效果。方法选取心力衰竭伴低钠血症62例,随机分为对照组与治疗组各31例,对照组采取常规抗心力衰竭治疗,治疗组在对照组基础上给予呋塞米持续静脉泵入加3.0%高渗盐水静脉滴注,持续治疗7 d后比较两组临床疗效、血钠水平以及心功能分级等。结果对照组血钠水平为(128.33±6.23)mmol/L,治疗组为(139.67±5.72)mmol/L,治疗组明显高于对照组(P<0.05)。治疗组心功能分级明显优于对照组(P<0.05)。治疗组总有效率为90.00%,对照组为67.50%,治疗组明显高于对照组(P<0.05)。结论对于心力衰竭伴低钠血症患者采取持续静脉泵入呋塞米联合静脉滴注高渗盐水治疗,效果显著,能明显改善患者血钠水平与心功能,具有临床应用价值。Objective To observe the clinical effect of continuous intravenous infusion of furosemide and hypertonic saline in the treatment of heart failure with hyponatremia. Methods Select 62 cases of hyponatremia associated with congestive heart failure,they were randomly divided into control group（ 31） and treatment group（ 31）,and the control group received conventional anti- heart failure treatment,and the treatment group received furosemide continuous intravenous infusion plus 3. 0% hypertonic saline infusion on the basis of the control group,compare the clinical efficacy,sodium levels and cardiac function,etc. Results Serum sodium levels in the control group was（ 128.33 ± 6.23） mmol / L,and the treatment group was（ 139.67 ± 5.72） mmol / L,so the treatment group was significantly higher（ P〈0.05）. And the treatment group was more excellent than the control group in NYHA classification（ P〈0.05）. The observation group total effective rate was90. 00%,and the control group was 67. 50%,and the observation group was significantly higher（ P〈0.05）. Conclusion The effect is significant to take continuous intravenous infusion furosemide combined with infusion of hypertonic saline treatment for the patients of heart failure and hyponatremia,and it can significantly improve cardiac function and serum sodium levels in patients with clinical value,and it has clinical value.
