摘　　要：目的提高静脉用药调配中心(PIVAS)药品的调配质量,促进临床合理用药。方法对2014-2015年医院PIVAS出现的各种类型差错进行统计分析,并提出相应的改进措施。结果 PIVAS主要差错按人员分为药师因素、医师因素和护师因素,具体为不合理医嘱、医嘱传递差错、审方差错、摆药差错、配置差错等,对各类差错实施相应改进措施后,各个环节的差错明显减少。结论规范并持续改进PIVAS操作流程可减少差错的发生,有利于促进医院的合理用药水平的提高。Objective This analysis was to improve the quality of drug dispensing in the pharmacy intravenous admixture services（ PIVAS） and to promote clinical rational drug usage. Methods Various types of errors from 2014 to 2015 were analyzed deeply for investigating the reasons and the corresponding measures. Results The types of errors include Pharmacist factor,physician factor and nurse factor which according by type of staff and the specific errors were irrational prescriptions,trial error,dispensing error,configuration error and so on. The amount of error from each procedure was reduced significantly. Conclusion There are some ways to reduce the occurrence of errors and improve the level of rational administration.
