摘  要:目的综合评估维生素D受体基因BsmI(rs1544410)多态性与冠心病易感性之间的关系。方法计算机检索Pub Med、Web of Science、CNKI、Wan Fang Data、VIP和CBM数据库,搜集维生素D受体基因BsmI(rs1544410)多态性与冠心病易感性的相关病例-对照研究,检索时限均为从建库至2016年5月。由两位研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Rev Man 5.3软件进行Meta分析。结果共纳入7个病例-对照研究,包括2 182例冠心病患者和5 925例对照人群。Meta分析结果显示:总人群中BsmI(rs1544410)B等位基因及BB基因型与冠心病的患病密切相关[Bvs.b:OR=1.36,95%CI(1.03,1.79),P=0.03;BBvs.bb:OR=1.70,95%CI(1.06,2.72),P=0.03;BB+Bb vs.bb:OR=1.52,95%CI(1.00,2.30),P=0.05]。按年龄进行的亚组分析结果显示:BsmI(rs1544410)多态性与年龄<65岁组冠心病患病风险密切相关[Bvs.b:OR=1.65,95%CI(1.00,2.73),P=0.05;BBvs.Bb+bb:OR=1.79,95%CI(1.08,2.97),P=0.02;BBvs.bb:OR=2.64,95%CI(1.12,6.25),P=0.03];按种族进行的亚组分析结果显示:BsmI(rs1544410)多态性与高加索人冠心病患病风险密切相关[Bvs.b:OR=1.47,95%CI(1.10,1.97),P=0.01;BB+Bb vs.bb:OR=1.71,95%CI(1.09,2.68),P=0.02;BBvs.Bb+bb:OR=1.39,95%CI(1.01,1.92),P=0.05;BBvs.bb:OR=1.80,95%CI(1.10,2.95),P=0.02];按基因检测方法进行的亚组分析结果显示:Taq Man方法检测组中BsmI(rs1544410)多态性与冠心病患病风险密切相关[Bvs.b:OR=2.18,95%CI(1.06,4.45),P=0.03;BB+Bb vs.bb:OR=3.32,95%CI(1.06,10.40),P=0.04;BBvs.bb:OR=3.31,95%CI(1.06,10.30),P=0.04];按病例组诊断方法进行的亚组分析结果显示:心电图组中BsmI(rs1544410)多态性与冠心病患病风险密切相关[Bvs.b:OR=1.15,95%CI(1.02,1.29),P=0.02;BB+Bb vs.bb:OR=1.22,95%CI(1.02,1.45),P=0.03;BBvs.bb:OR=1.31,95%CI(1.03,1.67),P=0.03]。结论维生素D受体基因BsmI(rs1544410)B等位基因可能增加冠心病的患病风险,尤其是高加索人和年龄<65岁的人群。Objective To evaluate the association between the Single Nucleotide Polymorphism (SNP) BsmI (rs1544410) in the vitamin D receptor gene and the susceptibility of coronary artery disease. Methods Databases including PubMed, Web of Science, CNKI, WanFang Data, VIP and CBM were searched from inception to May, 2016 to collect case-control studies about SNP BsmI (rs 1544410) in the vitamin D receptor gene and the susceptibility of coronary artery disease. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. Then Meta-analysis was performed by using RevMan 5.3. Results A total of seven studies were included, which involved 2 182 patients and 5 925 controls. The results of meta-analyses showed that the B allele and BB genotype in rs1544410 was associated with the risk of coronary artery disease (B vs. b: OR=1.36, 95%CI 1.03 to 1.79, P=0.03; BB vs. bb: OR=l.70, 95%CI 1.06 to 2.72, P=0.03; BB+Bb vs. bb: OR=1.52, 95%CI 1.00 to 2.30, P=0.05). Subgroup analysis by age showed that rs1544410 was associated with the risk of coronary artery disease in the age〈65 (B vs. b: OR=1.65,95%CI 1.00 to 2.73, P=0.05; BB vs. Bb+ bb: OR=1.79, 95%CI 1.08 to 2.97, P=0.02; BB vs. bb: OR=2.64, 95%CI 1.12 to 6.25, P=0.03). Subgroup analysis by ethnicity showed that rs1544410 was associated with the risk of coronary artery disease in Caucasians (B vs. b: OR=1.47, 95%CI 1.10 to 1.97, P=0.01; BB+Bb vs. bb: OR=1.71, 95%CI 1.09 to 2.68, P=0.02; BB vs. Bb+bb: OR=1.39, 95%C! 1.01 to 1.92, P=0.05; BB vs. bb: OR=l.80, 95%CI 1.10 to 2.95, P=0.03). Subgroup analysis by genotyping methods showed that rs1544410 was associated with the risk of coronary artery disease in the TaqMan (B vs. b: OR=2.18, 95%CI 1.06 to 4.45, P=0.03; BB+Bb vs. bb: OR=3.32, 95%CI 1.06 to 10.40, P=0.04; BB vs. bb: OR=3.31, 95%CI 1,06 to 10.30, P=0.04).
