胰岛素控制血糖治疗糖尿病合并肺结核的疗效分析

摘　　要：目的：探讨胰岛素控制血糖治疗肺结核合并糖尿病的疗效。方法选择2010年1月至2014年5月79例肺结核合并2型糖尿病患者，将其随机分为胰岛素组40例，口服降糖药组39例，两组均给予3HRZE/15HRE抗结核治疗，胰岛素组皮下注射胰岛素控制血糖，口服降糖药组口服降糖药控制血糖。结果胰岛素组2例因依从性差退出试验，口服降糖药组有1例因严重抗结核药物不良反应退出试验。胰岛素组空腹及餐后血糖均低于口服降糖药组，胰岛素组临床症状缓解时间低于口服降糖药组，胰岛素组痰菌阴转率、病灶吸收好转率、空洞闭合或缩小率明显高于口服降糖药组（ P﹤0.05）。结论肺结核合并糖尿病患者的治疗，血糖控制是关键，胰岛素能够良好地控制血糖，对于患者的临床症状缓解、抗酸杆菌阴转、肺部病灶吸收具有积极作用。Objective To investigate the clinical efficacy of insulin control blood glucose in the treatment of diabetes complicated with pulmonary tuberculosis. Methods Seventy-nine cases of pulmonary tuberculosis patients with type 2 diabetes mellitus were chosen from January 2010 to May 2014 ,they were randomly divided into the insulin group of 40 cases and the oral hypoglycemic drug group of 39 ca-ses. Both groups were given 3HRZE/15HRE treatment,the insulin group were treated with subcutaneous injection of insulin to control blood glucose,the oral hypoglycemic drug group were treated with oral med-ication. Observed the treatment outcomes of the two groups. Results In the insulin group,fasting and postprandial blood glucose were less than those in oral hypoglycemic drug group,the clinical symptom remission time was lower than that in the oral hypoglycemic drug group,the sputum negative conversion rate,focus absorption improvement rate and cavity closure or reduced rate were significantly higher than those of the oral hypoglycemic drug group（ P﹤0.05 ）. Conclusions In the treatment of diabetes complicated with pulmonary tuberculosis,blood glucose control is the key. Insulin can control blood glucose well,and it has a positive effect to relieve the clinical symptoms of patients,acid fast bacilli turn nega-tive,absorption of lung lesions.
