摘  要:目的:探讨阿魏酸钠对尘肺病患者肺纤维化指标I型胶原肽(CoⅠ)、Ⅲ型前胶原肽(PⅢP)的影响及其作用机制。方法:70例尘肺病患者按随机数字表法随机分为治疗组和对照组,各35例。所有患者均给予常规治疗,同时喷雾吸入4%克矽平,每周6次,每次10 m L,每次吸入约30min。治疗组在对照组治疗基础上加服阿魏酸钠片,3次/日,2片/次。疗程为6个月。观察两组患者治疗前后临床症状、血气分析指标、肺纤维化指标、D-二聚体、内皮素(ET)的变化情况。结果:两组患者临床症状、血气分析指标、肺纤维化指标均较治疗前得到明显改善(P<0.05)。治疗组患者治疗后临床总有效率、血气分析指标、肺纤维化指标CoⅠ和PⅢP、D-二聚体、ET均较对照组显著改善(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:阿魏酸钠能有效改善尘肺病患者肺通气功能,抑制肺纤维化,其主要作用机制可能是通过拮抗ET病理效应抑制肺部胶原蛋白的合成。AIM:To observe the effect and its mechanism of sodium ferulate for pulmonary fibrosis indexes such as Co I and PⅢP,on patients with pneumoconiosis.METHODS:70 pneumoconiosis patients were randomly divided into the treatment group and the control group,each with 35 patients.All patients were given conventional treatment with spray-inhalation of 4% polyvinylpyridine-N-oxide,10 mL,30 min per time,6 times per week.An extra 6-month treatment of sodium ferulate tablets were given to the treatment group,2 tablets per time,3 times per day.Changes of clinical symptoms,blood gas analysis index,pulmonary fibrosis index,D-dimer,ET(endothelin) were monitored before and after treatment in the two groups.RESULTS:Clinical symptoms,blood gas analysis index and pulmonary fibrosis index of the two groups were significantly improved than those of before treatment (P 〈 0.05).The clinical total effective rate,blood gas analysis index and pulmonary fibrosis indexes such as Co Ⅰ and PⅢP,D-dimer,ET in the treatment group were significantly improved compared with the control group (P 〈 0.05).There was no significant difference in adverse reactions between the two groups (P 〉 0.05).CONCLUSION:Sodium ferulate can effectively improve the pulmonary ventilation function and inhibit pulmonary fibrosis of patients with pneumoconiosis.To inhibit the synthesis of collagen by inhibiting the pathological effects of ET can be the possible underlying mechanism.
