摘　　要：目的探讨糖尿病慢性肾脏疾病(CKD)患者血清色素上皮衍生因子(PEDF)水平变化及其临床意义。方法 选取CKD患者152例,根据24 hUAER分为正常白蛋白尿组(NA组)、微量白蛋白组(MA组)和临床白蛋白尿组(PR组)。另选取健康人群(NC组)38名。检测各组PEDF、血管内皮因子(VEGF)、HbA_1c、C-RP、FPG等水平。结果 PR组PEDF水平高于其他组。各组SBP、PEDF、C-RP、24 hUAER、VEGF比较,差异均有统计学意义(P<0.05)。相关分析显示,血清PEDF与C-RP、HbA_1c、FPG、UAER和VEGF呈正相关(r=0.302、0.207、0.382、0.189、0.523,P<0.05);多元线性逐步回归分析显示,24 hUAER和C-RP是血清PEDF独立相关因素(β=0.61、0.18,P<0.05)。结论 CKD患者血清PEDF水平升高,其升高可能与CKD的发生发展有关。Objective To investigate the value of serum pigment epithelium derived factor （PEDF） in T2DM patients with kidney disease. Methods A total of 152 patients with CKD were enrolled in this study and divided into three groups according to their 24 h urinary albumin excretion rate （UAER）：normal albuminuria group （NA group） ,microalbumin group （MA group） ,clinical albuminuria group （PR group） . And 38 healthy control were also selected as control group. PEDF ,vascular endothelial growth factor （VEGF） ,glycated hemoglobin （HbA1c） ,C‐reactive protein （C‐RP） ,and fasting plasma glucose （FPG） were measured in all subjects. Results PEDF level was highest in PR group than in the other groups.SBP ,PEDF ,C‐RP ,UAER ,VEGF were significantly different in each group （ P〈 0.05 ）. Association analysis showed that serum PEDF was positively correlated with C‐RP ,HbA1c ,FPG ,UAER ,and VEGF （r=0.302 ,0.382 ,0.189 ,0.523 ,0.207 ;P〈0.05）. Multivariate linear stepwise regression analysis showed that UAER and C‐RP were independent related factors for serum PEDF （β=0.61 ,0.18 ;P〈 0.05 ）.Conclusion Serum PEDF level increases in patients with CKD ,and may contribute to the pathogenesis of CKD.
