腹腔镜子宫全切术两种镜下阴道残端缝合方法的疗效对比

摘　　要：目的:比较两种缝合方式对腹腔镜子宫全切术后阴道残端愈合的影响,探讨腹腔镜全子宫切除术阴道残端的处理方法。方法:选择2012年6月至2014年12月收治的拟行腹腔镜子宫全切术的80例患者作为研究对象,随机分为"U"字缝合组(n=40)与连续缝合组(n=40),比较两组患者术中阴道残端处理时间、阴道流血量、住院时间、术后阴道残端出血、息肉发生率等随访情况。结果:两组阴道处理时间、住院时间差异无统计学意义(P>0.05)。"U"字缝合组较连续缝合组术后残端出血、息肉发生率低,差异有统计学意义(P<0.05)。结论:两种方法均适于腹腔镜全子宫切除术,但腹腔镜下行"U"字缝合具有术后阴道出血少、息肉发生率低等优点,更利于妇科微创手术的临床应用。Objective：To compare the effect of two suture methods of vaginal stump in laparoscopic hysterectomy on stump union,and discuss the treatment of vaginal stump after laparoscopic total hysterectomy. Methods： Eighty patients underwent laparoscopic hysterectomy from Jun.2012 to Dec.2014,and were selected as research object. They were randomly divided into ＂U＂ suture group（ experimental group） and continuous suture group（ control group）. The time of vaginal stump processing,vaginal blood loss,the hospital stay,postoperative vaginal stump bleeding and polyp were compared between the two groups. Results： The differences of the time of vaginal stump processing and the hospital stay were not statistically significant between ＂U＂ suture group and continuous suture group（ P〈0. 05）. The incidence of vaginal stump bleeding and ployp in＂U＂suture group was significantly lower than that in continuous suture group（P〈0. 05）. Conclusions： ＂U＂ suture and continuous suture are both suitable for laparoscopic total hysterectomy,while the＂U＂suture has advantages of the less postoperative vaginal blood loss and fewer polyp,it is favorable for carrying on gynecologic minimally invasive surgery.
