腹腔镜术中超声在妇科手术中的应用价值

摘　　要：目的:探讨腹腔镜超声在妇科手术中定位诊断的临床价值。方法:收集分析2012年6月至2014年12月42例行妇科腹腔镜手术并行术中超声检查患者的临床资料。术中均采用腹腔镜超声查找残存病灶,并用单极电钩进行定位,帮助术者准确切除病灶,再将术中超声检查结果与术前经腹超声或经阴道超声结果进行比较。结果:与术前经腹超声或经阴道超声结果相比,42例患者中24例存在残存病灶,均定位成功剥除;9例患者的诊断与术前诊断不同,经术中探查病灶后,均更正了术前诊断。结论:术中行腹腔镜超声可指导妇科医师有效地清除病灶,弥补术前经腹超声或经阴道超声的不足。术中进一步确定病灶位置,指导手术进行探查,降低了术后残存率与复发率,提高了手术疗效,具有不可替代的作用。Objective： To explore the clinical value of laparoscopic ultrasound for localization diagnosis in gynecologic surgery.Methods： The clinical data of 42 patients undergoing gynecologic laparoscopic surgery and intraoperative ultrasound from Jun. 2012 to Dec. 2014 were collected and analyzed. The patients all underwent laparoscopic ultrasound during surgery to locate the residual lesions,and monopolar electrical hook was used for localization to help the surgeon to precisely resect lesions. Then the result of intraoperative ultrasound inspection and that of preoperative abdominal ultrasound or transvaginal ultrasound inspection were compared. Results： Compared with the result of preoperative abdominal ultrasound or transvaginal ultrasound,24 of 42 patients had residual lesions,which were all successfully located and removed; in the 42 selected patients,the diagnoses of 9 patients were different from their preoperative diagnoses,after intraoperative exploration of the lesions,their preoperative diagnoses were corrected. Conclusions： Intraoperative laparoscopic ultrasound can provide guidance for gynecologists to effectively remove the lesions,make up for the deficiency of preoperative abdominal ultrasound or transvaginal ultrasound,further locate the lesions during surgery,guide surgical resection and exploration,reduce the residue rate and the recurrence rate after surgery,and increase the efficacy of surgical treatment,which is of irreplaceable application value.
