杂交技术治疗腹股沟嵌顿疝7例分析

摘　　要：目的：评价杂交技术在腹股沟嵌顿疝手术治疗中的临床效果。方法：选取上海市中医药大学附属普陀医院2014年2月至2016年2月应用杂交技术治疗的7例腹股沟嵌顿疝患者,均为男性,27岁-81岁,平均（65.0±19.24）岁,均为单侧斜疝嵌顿,其中1例为传统修补后的复发疝,3例合并糖尿病、高血压、冠心病等内科疾病。嵌顿时间6-18 h,平均（8.0±4.43）h。分析手术时间、住院时间、术后并发症等临床资料。结果：嵌顿疝内容物均为小肠。全组患者均无死亡,手术时间65-150 min,平均（85.0±29.44）min。术后腹股沟区肿胀6例,阴囊水肿4例,血清肿2例。无切口感染、缺血性睾丸炎及腹股沟区顽固性疼痛病例。肿胀及血清肿经对症理疗后2-8周内症状基本消失,均未行穿刺引流。平均住院（5.0±1.82）d,随访2-24个月未见复发。结论：杂交技术治疗腹股沟嵌顿疝具有安全、有效、康复快等优点,在严格掌握适应证前提下可应用于临床。Objective： To evaluate the efficacy of hybrid procedure in incarcerated inguinal hernia. Methods： Seven patients who underwent hybrid procedures for incarcerated inguinal hernia from Feb. 2014 to Feb. 2016 were selected. 7 cases were all males aged 27-81 years,with an average age of（ 65. 0 ± 19. 24） years. All cases were diagnosed with unilateral incarcerated indirect hernia,one of them was recurrent hernia after traditional repair,and 3 patients combined with diabetes,hypertension and coronary heart disease. Embedded time was 6-18 h,the average time of incarceration was（ 8. 0 ± 4. 43） h. The operation time,hospital stay,postoperative complications and other clinical data were analyzed. Results： Embedded contents were all small intestine. Operative time ranged from 65 to 150 min with an average of（ 85. 0 ± 29. 44） min. There were no postoperative complications such as incision infection,ischemic orchitis or intractable pain in inguinal area,but swelling of inguinal area occurred in 6 cases,and scrotum edema occurred in 4 cases,seroma in 2 cases. Swelling and seroma disappeared in 2-8 weeks after physical therapy without puncture drainage. The average hospital stay was（ 5. 0 ± 1. 82） d. No hernia recurrence was found in a short-term follow-up of 2 months to 24 months. Conclusions： Hybrid procedure is a safe and effective technique with fast recovery,and can be used in highly-selected patients who suffered from incarcerated inguinal hernia.
