肺隔离症的外科诊疗分析

摘　　要：目的总结肺隔离症(PS)的诊断和外科治疗经验,以提高对本病的诊疗水平。方法回顾性分析我院15例PS患者的临床资料。结果本组病例术前明确诊断11例,其中10例行增强CT+三维重建,3例行CT血管造影(CTA),所有病例均行开胸或胸腔镜手术,术后病理证实为PS。全组病例均顺利出院,失访2例,余随访3个月～5年,症状完全消失。结论胸部增强CT+三维重建及CTA已成为PS重要的诊断方法,胸腔镜或开胸手术疗效均满意,手术的关键在于处理滋养血管及分离粘连。ObjectiveTo summarize the clinical characteristics of pulmonary sequestration（PS）in 15 patients,and to improve the preoperative diagnosis rate and treatment of PS.MethodsWe retrospectively reviewed 15 patients with a pathologically proven diagnosis of PS in our hospital between Jan 2009 and Jan 2016.ResultsIn this group,11 cases were diagnosed preoperatively,and 10 cases were examined by Contrast-enhanced CT and/or CT angiography（CTA）. All cases were treated by thoracotomy or VATS. Follow-up was performed in 13 cases,and no recurrence was founded.ConclusionsClinical manifestations of PS varied. Contrast-enhanced CT or CTA is the preferred imaging modality for preoperative diagnosis. VATS or thoracotomy has been suggested for PS,and the key points are the handling of supply arteries and the dissection of adhesions.
