摘　　要：目的探讨24 h动态收缩压（SBP）与老年人群臂踝脉搏波传导速度（ba PWV）的关联性。方法 2006～2007年第1次对开滦集团公司在职及离退休职工进行健康体检,此后分别于2008～2009、2010～2011年进行第2、3次健康体检,并于第3次体检时整群随机抽取年龄≥60岁的开滦集团离退休员工2814例进行动态血压监测、ba PWV测定等检查。符合入选标准的为2464例,资料完整者1004例。采用多因素线性回归分别分析24h SBP、日间SBP、夜间SBP与ba PWV的关联性。结果 （1）按照24h SBP、日间SBP、夜间SBP三分位数分别将研究对象分为3组,随着24h SBP、日间SBP、夜间SBP升高,ba PWV均增加（均P〈0.001）。（2）多因素线性回归分析显示：校正混杂因素后,24h SBP、日间SBP、夜间SBP每增加1个标准差,ba PWV分别增加81.15、73.18、81.30 cm/s;此外,诊室SBP也与ba PWV呈线性正相关,且回归系数大于动态SBP。结论 24h SBP、日间SBP、夜间SBP与ba PWV呈正相关,但关联性弱于诊室SBP。Aim To investigate the correlation between the 24 hours ambulatory systolic blood pressure（ SBP）and brachial-ankle pulse wave velocity（ ba PWV） in the elderly. Methods The first health examination for in-service and retired staff in Kailuan Group Company was performed from 2006 to 2007,with the second,third health examination from 2008 to 2009,2010 to 2011 respectively. At the third health examination,a cluster random sample of 2814 cases ≥60 years old and retired employees was selected,and ambulatory blood pressure,ba PWV,etc. were examined. 2464 cases met the inclusion criteria,and 1004 cases had complete data. Multivariate linear regression analysis was used to analyze the correlation between 24 h SBP,day SBP,night SBP and ba PWV. Results（ 1） Research subjects were divided into three groups according to 24 h SBP,day SBP and night SBP tertiles. ba PWV was increased with the increase of 24 h SBP,day SBP and night SBP（ all P〈 0. 001）.（ 2） Multivariable linear regression analysis showed that when24 h SBP,day SBP,night SBP increased 1 standard deviation,Ba PWV was increased by 81. 15,73. 18,81. 30 cm / s,after adjustment for confounding factors. In addition,the diagnosis room SBP also showed a linear positive correlation with ba PWV,and the regression coefficient of diagnosis room SBP was larger than that of ambulatory SBP. Conclusion24 h SBP,day SBP and night SBP are positively correlated with ba PWV,but the correlation was weaker than that of diagnosis room SBP.
