踝关节骨折伴三角韧带损伤应用手术治疗的临床分析

摘　　要：目的观察踝关节骨折伴三角韧带损伤采用手术治疗的临床疗效。方法选择2001年1月至2014年12月期间我院收治的251例踝关节骨折伴三角韧带损伤患者为研究对象,均对断骨实行切开复位内固定,而损伤三角韧带则行切开探查修复来恢复踝关节内结构的稳定。手术后进行X线检查,并根据美国骨科足踝协会(AOFAS)制定的踝-后足评分和视觉模拟评分(VAS)来评定手术前后踝-后足功能评分以及疼痛评分,了解疗效以及骨折愈合时间。结果在251例患者中,优为224例,良为20例,优良率为97.21%;平均骨折愈合时间(12.6±1.9)周。术后AOFAS评分与术前比较明显升高[(92.48±5.29)分vs(30.48±5.11)分],VAS评分与术前比较明显降低[(1.83±1.44)分vs(8.63±1.12)分],差异具有统计学意义(P<0.05)。结论采用手术方案治疗踝关节骨折合并三角韧带损伤的患者是临床中最佳的治疗方案,优良率高,术后患者踝关节功能恢复效果佳,疼痛得到大幅缓解。Objective To observe the clinical efficacy of surgical treatment for ankle fractures complicated with deltoid ligament damage. Methods 251 cases of patients with ankle fractures complicating deltoid ligament damage treated in our hospital from January2001 to D ecember 201 4 were selected as research subjects. The broken bones were implemented open reduction and internal fixation（ORIF）, the injured deltoid ligament was implemented open prostheses for the recovery of structure stability in ankle. The X-ray was performed after surgery, and the scores of ankle-hindfoot and V A S were evaluated by A nkle-H indfoot Scale and V isual A nalogue Scale formulated by AOFAS, the efficacy and fracture healing time were recorded. Results The excellent and good rate of 251 patients was97.21 %, with best in 224 cases, good in 20 cases; the average fracture healing time was（12.6 ± 1.9） weeks. A fter surgery, the AOFAS score increased significantly [（92.48 ± 5.29） points vs（30.48 ± 5.11） points], and the V A S score decreased significantly [（1.83 ± 1.44） points vs（8.63 ± 1.1 2） points], with statistically significant difference（P〈0.05）. Conclusions Surgery is the best therapeutic schedule for patients with ankle fractures complicating deltoid ligament damage, with high excellent and good rate, good recovery of ankle joint function and sharp remission of pain.
