摘 要:目的基于3日饮食日记的干预,对腹膜透析患者的营养状态进行探讨。方法将60例腹膜透析患者按饮食治疗方式的不同分为3日饮食日记干预组(研究组)、普通饮食组(对照组),每组30例。研究组在护士和营养科医生的指导下,采用选择性的饮食治疗方案;对照组行普通饮食治疗方案。随访6个月,记录2组3d饮食,即磷摄入量、血浆白蛋白,检测血磷浓度等指标,并评价2组营养状况。结果随访第1个月和第6个月,研究组磷的摄入、总热量显著下降且低于对照组(P<0.05);白蛋白、血红蛋白、铁蛋白水平显著增加且高于对照组(P<0.05),血清磷水平降低且显著低于对照组(P<0.05);营养良好率显著高于对照组(64.28%比29.62%,P<0.05)。结论基于3日饮食日记的干预,结合临床症状和实验室化验结果,制订个性化饮食治疗方案,可以提高患者营养状态并改善高磷血症。Objective To investigate the effect of 3-day diet diary intervention on nutritional status of peritoneal dialysis patients.Methods Sixty peritoneal dialysis patients were divided into two groups,with 30 patients in each group.The 3-day diet diary intervention group(study group) received selective diet therapy under the nutrition guidance by nurses and physicians.The normal diet group(control group)received ordinary diet therapy.All patients were followed up for 6 months.Phosphorus intake and serum albumin and phosphorus concentrations were measured and nutritional status was evaluated in both groups.Results At the first and sixth month of followup,compared with control group,3-day diet diary intervention decreased phosphorus intake,total calories and serum phosphorus concentrations,increased serum albumin,hemoglobin and ferritin levels,and elevated good nutrition rate(64.28% vs 29.62%)(P〈0.05).Conclusion Personalized diet therapy regimen formulated according to 3-day diet diary intervention,clinical symptoms and laboratory test results can improve nutritional status and attenuate hyperphosphatemia in peritoneal dialysis patients.
