顺行髓内针内固定治疗第五掌骨颈骨折的临床分析

摘　　要：目的评价顺行髓内针内固定治疗掌骨颈骨折的临床疗效。方法闭合性第五掌骨颈骨折29例,采用闭合复位顺行髓内针内固定治疗,分别从骨折愈合时间、术后手指关节功能恢复情况、并发症的发生进行评价疗效。结果术后均获得6~12个月（平均9.5个月）随访,骨折愈合时间为（3.72±1.62）周,按手指关节总活动度（TMA）评定功能：优20例,良8例,可1例,优良率为96.7%,1例出现一过性尺神经手背支麻痹,2例针尾皮肤激惹。结论顺行髓内针内固定治疗掌骨颈骨折疗效满意,操作简单,创伤小,并发症少,费用低廉,值得推广应用。Objective To evaluation the clinical effect of antegrade intramedullary fixation in the treatment of the fifth metacarpal neck fracture. Methods A total of 29 patients with the fifth metacarpal neck fracture were treated with closed reduction and intramedullary nail internal fixation. The clinical efficacy was evaluated respectively according to the fracture healing time,postoperative finger joint function recovery,complications. Results After operation,patients were followed-up for 6 months to 12 months（ average of9. 5 months）; the fracture healing time were（ 372 ± 1. 62） weeks; according to the total activity of finger joints（ TMA） evaluation function： excellent in 20 cases,good in 8 cases,common in 1 cases,the excellent and good rate was 96. 7%; dorsal branch of ulnar nerve paralysis in 1 case,needle tail skin induced provoke in 2 cases. Conclusion Antegrade intramedullary fixation used in the treatment of neck of metacarpal bone fracture curative effect is satisfactory,with simple operation,small trauma,less complications,low cost. It is worthy of popularization.
