摘 要:目的探讨胰岛素(INS)对糖尿病大鼠膀胱逼尿肌病理组织形态学表现的影响。方法使用链脲佐菌素(STZ)建立诱导糖尿病大鼠模型,按照随机原则分为INS注射组(28例)与实验观察组(28例),且设置正常对照组(28例)。在7、11周时分别对上述3组大鼠进行膀胱逼尿肌切片标本常规HE染色,并且观察大鼠膀胱逼尿肌在光镜下的组织学形态,并随机选择一些膀胱逼尿肌切片在电镜下进行超微病理检查。结果在光镜下观察的HE染色石蜡切片中,糖尿病观察组的膀胱逼尿肌肌束之间排列疏松紊乱,间隙显著变宽,胶原组织含量减少,在11周时Vh7周时更明显;而对照组的膀胱逼尿肌横断切面和纵行切断面所观察的肌束之间排列非常整齐,肌束之间的结构连接紧密,间隙中有丰富的结缔组织;INS注射组与对照组比较无明显差异。在电镜下观察HE染色石蜡切片中,糖尿病观察组肌束内平滑肌细胞(SMC)的细胞直径变大,间隙变小,胶原纤维含量减少,SMC肌膜致密带密度减小,密度带之间分界不清晰,肌膜各区域的密度逐渐相同,肌细胞的中间连接、密斑变少,但仍然可观察到神经;上述改变较少随着时间的增加而改变;INS注射组和对照组的超微结构大体相同。结论糖尿病大鼠早期即有膀胱逼尿肌组织形态学上的变化,早期注射INS治疗可以对预防和减轻上述变化有良好的效果,可能与INS在预防和延缓糖尿病神经原膀胱的发生、发展方面有关,提示临床医生对临床上已经诊断出患有早期糖尿病神经原膀胱的患者应尽早使用INS治疗。Objective To study the insulin ( INS ) on diabetic rats bladder tissue morphology performance forced urinous myopathy. Methods Use chain urea with cephalosporins ( STZ ) induced diabetic rats model was established, based on the principle of random divided into insulin injection group ( 28 cases ) and the experimental group ( 28 cases ) , and set up the normal control group ( 28 cases ) .At 7, 11 w, respectively, for the above three groups of rats bladder detrusor biopsy specimens of routine HE staining, and observed the rat bladder detrusor histological morphology under light microscopy, and randomly selected some of the bladder detrusor slice ultrastructural examination under the electron microsco.Results Under light microscopy observation of paraffin section, HE dyeing diabetes group between the bladder detrusor muscle bundles of loosely arranged disorder, gap widened significantly, reduce collagen content, in the 11 w was more obvious than when 7 w; And the control of the bladder detrusor cross sectional plane and longitudinal cut surface was observed between muscle bundle arrangement was very neat, the structure of the connection between muscle bundles were tight, clearance was rich in the connective tissue; .Insulin injection group compared with the control group had no obvious difference.HE dyed in paraffin section, when observed under electron microscope, diabetes group within the muscle bundle of smooth %- muscle cells ( SMC ) had larger cell diameter, smaller gap, and collagen fiber content reduce, SMC muscle membrane density decreased density, density zone between the boundary was not clear, the density of various regions gradually the same muscle membrane, muscle cells in the middle of the connection, tight spot less, but still can be observed nerve; The above change little with the increase of time and change; Ultrastructure of insulin injection group and the control group in general were the same. Conclusions Are laid down early diabetic rats bladder detrusor change of tissue
