住院患者Rh表型血清学分析及临床应用

摘　　要：目的了解浙江地区住院患者Rh表型分布规律，建立Rh表型库为本地区血液中心合理备血、建立稀有表型血源资料库提供依据。方法用Rh表型微柱凝胶分型卡测定Rh表型D、C、E、c、e。结果经统计分析浙江地区住院患者有效数据29338例，D、C、E抗原的阳性率分别为99．54％、89．51％和47．56％，Rh阳性表型分布规律为CCDee〉CcDEe〉CcDee〉ccDEE〉ccDEe〉CCDEe〉CcDEE〉ccDee〉CCDEE，Rh阴性表型分布规律Ccdee〉ccdee〉CCde〉ccdEe〉ccdEe。结论掌握本地区住院患者的Rh血型系统表型的分布状况，为血液中心及时提供Rh配型相舍的血液，防止高频抗体的产生，对保障合理、安全用血具有重要临床意义。Objective To study the distribution of Rh blood group system of Zhejiang area and build Rh phenotype library, helping local blood center to establish rare phenotype blood library, and guide clinical reasonable blood preparation. Methods Using micro column gel cards to detect transfusion patients＇ Rh phenotypes including D, C, E, c, and e. Results 29338 inpatients in Zhejiang area were effective and analysed, Data showed that positivity rate of D antigen was 99.54%, meanwhile C antigen was 89.51% and E antigen was 47.56%. The phenotype distribution in RhD positive was CCDee〉 CcDEe〉 CcDee〉 ccDEE〉 ccDEe〉 CCDEe〉 CcDEE〉 ccDee〉 CCDEE, however in RhD negative phenotype was Ccdee〉 ccdee〉 CCdee〉 ccdEe〉 CcdEe. Conclusion We showed the distribution of Rh blood group system in Zhejiang area, to provide the data basis for reasonable blood usage and Rh matched blood supply in time, avoid the generation of high-frequency antibodies. These results have important clinical significanees for ensuring the reasonable and safety blood usage.
