摘 要:目的探讨子宫内膜息肉(endometrial polyp,EP)的数目、大小和位置与异常子宫出血间的关系。方法回顾性分析2010-08-2014-02我院行宫腔镜检查且经病理检查证实为子宫内膜息肉的患者542例,分析息肉的数目、大小和位置与异常子宫出血的关系。结果 542例子宫内膜息肉患者,219例(40.41%)有异常子宫出血症状,86例(15.87%)为不孕,无症状者237例(43.73%)。219例异常子宫出血患者中,单发息肉与多发息肉并发异常子宫出血的发生率分别为34.47%(111/322)和49.09%(108/220),差异有统计学意义(χ^2=11.600,P〈0.01)。322例单发息肉患者中,直径≤10 mm和直径〉10 mm息肉并发异常子宫出血的发生率分别为26.98%(34/126)和39.29%(77/196),差异有统计学意义(χ^2=5.138,P=0.023)。542例内膜息肉患者中,不同部位所致异常子宫出血的发生率比较,差异无统计学意义(χ^2=0.715,P〉0.05)。结论单发息肉直径〉10 mm或多发性息肉并发异常子宫出血的发生率增大,而息肉位置与异常子宫出血无明显关联。Objective To explore the relationship between the number,size and location of endometrial polyp( EP) and abnormal uterine bleeding. Methods A total of 542 cases of endometrial polyps collected from August 2010 to February 2014 who confirmed by hysteroscopy in our hospital were enrolled in this study. The relationships between the number,size and location of endometrial polyps and abnormal uterine bleeding were retrospectively analyzed. Results Of 542 cases,there were 219 cases(40.41%) of abnormal uterine bleeding,86 cases(15.87%) of infertility,237 cases(43.73%) of asymptom. In 219 cases of abnormal uterine bleeding,the incidences of solitary polyp and multiple polyps with abnormal uterine bleeding were 34. 47%(111/322) and 49. 09%(108/220)respectively,and there was statistical difference(χ^2= 11. 600,P〈0. 01). In 322 cases of solitary polyp,the incidences of small polyp( ≤10mm) and large polyp(10mm) with abnormal uterine bleeding were 26. 98%( 34 /126) and 39. 29%( 77 /196) respectively,and there was significant difference(χ^2= 5. 138,P= 0. 023). In 542 case of endometrial polyp,the incidence of abnormal uterine bleeding had no significant difference between different parts of endometrial polyp( χ^2= 0. 715,P〈0. 05). Conclusion Bigger solitary polyp( 10 mm) or multiple polyps have a higher incidence of abnormal uterine bleeding. Abnormal uterine bleeding may be not related to the location of endometrial polyp.
