摘 要:目的探讨MSCT后处理技术结合MRI在胫骨平台骨折诊断和治疗方案选择中的临床指导作用。方法收集经临床手术证实的36例胫骨平台骨折的X线平片、MSCT重组图像及MRI检查资料,根据Schatzker分型法对胫骨平台骨折进行分型诊断,并与术中所见进行对照、分析。结果 X线平片诊断Schatzker I型骨折2例,II型13例,III型3例,IV型4例,Ⅴ型6例,VI型4例,诊断准确率为69.2%。MSCT后处理技术与MRI相结合诊断I型0例、II型18例、III型2例、IV型3例、Ⅴ型7例、Ⅵ型6例,诊断准确率达100%。结论 MSCT后处理技术与MRI结合对胫骨平台骨折手术前的骨折分型、治疗方案制订有重要的指导意义。Objective To explore the role of MSCT combined with MRI in diagnosis of tibial plateau fracture. Methods 36 cases of fractures of the tibial plateau confirmed by operation with the data of X-ray, MSCT post-processing imaging and combined with MRI were collected. Classification of tibial plateau fractures was performed according to the Sehatzker classification method, and compared with the findings in the operation. Results X-ray plain film diagnosed 2 cases of fracture of Schatzker Ⅰ:0, Ⅱ : 13,Ⅲ:3, Ⅳ :4, Ⅴ :6, Ⅵ:4, diagnostic accuracy of was 69.2% ( Schatzker Ⅰ: 2 cases, Ⅰ: 13cases,Ⅲ : 3cases, Ⅳ : 4 cases, Ⅴ : 6 cases, Ⅵ: 4 cases). MRI and MSCT post-processing techniques combined with MRI diagnosed Ⅰ: 0 case, Ⅱ: 8 cases,Ⅲ: 2 cases, Ⅳ: 3 cases, Ⅴ : 7 cases, Ⅵ : 6 cases, diagnostic accuracy of was 100% ( Sehatzker Ⅰ: 0 case, Ⅱ: 18 cases,Ⅲ: 2 cases, Ⅳ : 3 cases, Ⅴ : 7 cases, Ⅵ : 6 cases). Conclusion MSCT combined with MRI has important significance in the diagnosis of tibial plateau fracture classification, and formulating the treatment plan.
