腹腔镜联合药物治疗子宫内膜异位症合并不孕的临床分析

摘　　要：目的探讨腹腔镜联合药物治疗子宫内膜异位症合并不孕的临床效果。方法选取本院2013年1月~2015年1月期间收治的76例子宫内膜异位症合并不孕的患者作为研究对象,随机分为观察组与对照组,每组38例。对照组予腹腔镜手术治疗,观察组予腹腔镜手术联合药物治疗方法 ,对两组患者治疗的效果、术后并发症、血清CA125指标及术后1年内的妊娠情况进行比较。结果观察组治疗总有效率为97.37%,对照组治疗总有效率为81.58%,差异有统计学意义(P<0.05);观察组术后并发症发生率为2.63%,对照组术后并发症发生率为5.26%,两组比较,差异无统计学意义(P>0.05);两组治疗后血清CA125指标均低于治疗前,差异有统计学意义(P<0.05);观察组术后1年妊娠率为34.21%,对照组妊娠率为15.79%,观察组明显高于对照组,差异有统计学意义(P<0.05)。结论对子宫内膜异位症合并不孕的患者采用腹腔镜联合药物治疗效果显著,且术后1年内妊娠率较高,在临床中有推广应用的价值。Objective To analyze and discuss the clinical effects of laparoscope combined with drug therapy in treat- ment of endometriosis complicated by infertility. Methods 76 patients with endometriosis complicated by infertility treated in our hospital from January 2013 to January 2015 were selected and randomly divided into observation group and control group, each with 38 cases. Patients in the control group were given laparoscopic surgery, while patients in the observation group were given laparoscope combined with drug therapy. The treatment effects, postoperative compli- cations, serum CA125, and pregnancies in 1 year after operation were compared between two groups. Results As to the effects of treatment, the overall response rate in the observation group was 97.37%, which was 81.58% in the control group and the difference was statistically significant（P〈0.05）. As to the incidence rate of postoperative complications, the incidence rate in the observation group was 2.62%, while it was 5.26% in the control group, and there was no sig- nificant difference between two groups（P〉0.05）. The levels of serum CA125 in both groups were significantly lower after treatment than those befi^re treatment（P〈0.05）. The pregnant rate in the observation group was 34.21% within 1 year af- ter operation in the observation group, which was significantly higher than the 15.79% in the control group （P〈0.05）. Conclusion The clinical effect of laparoscope combined with drug therapy is significant in treatment of endometriosis complicated by infertility, with favorable pregnant rate within 1 year after operation, which is worthy to be promoted.
