腕关节镜辅助关节内韧带解剖重建治疗慢性下尺桡关节不稳定的临床应用

摘　　要：目的介绍自体半腱肌肌腱移植、腕关节镜辅助下远尺桡韧带解剖重建的适应证、手术方法及治疗慢性下尺桡关节不稳定的初期随访结果。方法选择2013年1月～2014年6月应用自体半腱肌肌腱移植、腕关节镜辅助下远尺桡韧带解剖重建术治疗10例慢性DRUJ不稳定患者。平均年龄23岁，男4例，女6例。给予腕关节镜辅助下取自体半腱肌腱行下尺桡关节内韧带解剖重建，术后采用改良Mayo评分评估腕关节功能，及手术结果满意情况。结果术前平均（67．2±1.3）分，术后平均（94．3±1．5）分，在治疗前后评分方面比较，差异有统计学意义（P〈0．05）。10例患者均对手术结果满意。结论采用腕关节镜辅助关节内韧带解剖重建手术治疗慢性下尺桡关节不稳定患者，具有较好的治疗效果，值得在临床上应用。Objective To introduce the indications and operation method of semitendinosus tendon autograft and wrist arthroscopy assisting intra-articular ligament anatomical reconstruction and the early follow-up result of its application in chronic distal radioulnar joint （DRUJ） instability. Methods 10 patients with chronic DRUJ instability were treated in our hospital from January 2013 to June 2014 by semitendinosus tendon autograft and wrist arthroscopy assisting intm- articular ligament anatomical reconstruction. The average age of the patients was 23 years, and 4 patients were male and 6 were female. The patients were given wrist arthroscopy assisting intra-articular ligament anatomical reconstruc- tion by semitendinosus tendon autograft. After operation, the wrist functions and satisfaction degrees of patients were e- valuated by modified Mayo scoring system. Results The average score before operation was （67.2±1.3）scores, and （94.3±1.5）scores after operation, showing significant differences before and after treatment （P〈0.05）. All of the 10 pa- tients were satisfied with the operation outcomes. Conclusion Wrist arthroscopy assisting intra-articular ligament anatomical reconstruction has favorable treatment effect in patients with chronic distal radioulnar joint instability, which is worthy to be promoted.
