磁共振对胎盘植入的诊断价值

摘　　要：目的探讨磁共振常规检查对胎盘植入的诊断价值。方法回顾性分析2011年1月～2014年12月间我院收治的磁共振影像学资料完整、临床可疑胎盘植入患者25例为研究对象，并与手术病理或临床综合诊断结果相比较，统计磁共振在鉴别胎盘粘连及植入的敏感性、准确性和特异性。结果胎盘滞留及粘连组10例中，磁共振正确诊断8例；胎盘植入及穿透组15例中，磁共振正确诊断13例。阳性预测值（epv）为86．2％（13／15）、阴性预测值（NPV）为80．0％（8／10），而检查的敏感性（Sen）为86．7％（13／15）、准确性（ACC）为84．0％（21／25）、特异性（Spe）为80．0％（8／10），统计学上显示MRI检测方法与临床最终确诊结果之间差异无统计学意义（X2=o．00，P〉0．05）。结论磁共振的软组织分辨率较高，可正确诊断胎盘植入，并准确判断胎盘植入深度及范围，以及子宫周围器官的受侵程度．在胎盘植入的临床诊断上具有重要的应用价值。Objective To explore the value of magnetic resonance imaging in the diagnosis of placenta implantation. Methods Selected January 2011 to December 2014 in our hospital during the period of magnetic resonance imaging data integrity and clinically suspected of placenta aeereta in 25 cases as the research object, were retrospectively analyzed, and compared with results of pathology or clinical comprehensive diagnosis, statistical magnetic resonance in the sensi- tivity, specificity and accuracy of identification of placenta adhesion and implantation. Results In 10 cases of placenta retention and adhesion group, 8 cases were diagnosed correctly by MRI. In 15 cases of placenta implantation and pene- tration group, 13 cases were diagnosed correctly by MRI. Positive predictive value （PPV） was 86.7%（13/15）, the neg- ative predictive value （NPV） was 80.0%（8/10）, and the sensitivity （Sen） was 86.7%（13/15）, the accuracy （ACC） was 84.0%（21/25）, and the specificity（Spe） was 80.0%（8/10）. There was no statistical significance between statistically significant MRI detection method with the final clinical diagnosis （X2=0.00,P〉0.05）. Conclusion Magnetic resonance soft tissue with higher resolution, correct diagnosis of placenta accreta, and accurate judgement of placenta implanta- tion depth and scope, and around the uterus organs involvement,in the clinical diagnosis of placenta accreta has im- portant application value.
