不同剂量依托咪酯对儿童全身麻醉血清S100β蛋白水平的影响

摘　　要：目的探讨不同剂量依托咪酯对儿童全身麻醉血清S100β蛋白水平的影响。方法选择2014年7月～2015年7月我院接诊的60例需进行下肢骨科手术的患儿，通过抽签法将患儿平均分为三组，三组患儿均使用依托咪酯进行全身麻醉。A组剂量0．4mg/kg，B组剂量0．5m∥kg，C组剂量0．6mg，kg，观察三组患儿在T0、T1、T2、T3、T4时脑电双频谱指数、MAP、HR及血清S100~蛋白水平的变化。结果三组患儿在不同时间点脑电双频谱指数均呈现不同程度变化，在T1．T2、T3、T4时，A组结果高于其他两组（P〈0．05）；三组患儿MAP、HR水平均呈现不规则的变化情况，A组患儿在172、T3、T4时结果大于其他两组（P〈0．05）；在T2、T3、T4时，A组患儿的血清S100β蛋白水平低于其余两组（P〈0．05）。结论使用0．4mg／kg剂量依托咪酯在麻醉中可以维持血流动力的稳定，不会对患儿的S100~蛋白水平造成影响，可有效保护患儿的中枢神经系统，减少认知功能出现障碍的发生率。Objective To discuss the influence of different doses of etomidate on S100β protein level in children with general anesthesia, nethods 60 children requiring lower limb orthopedic surgery treated in our hospital from July 2014 to July 2015 were selected and randomly and equally divided into three groups. Patients in three groups were all given general anesthesia by etcmidate. The dose of etomidate in group A was 0.4mg/kg, in group B was 0.5 mg/kg, and in group C was 0.6 mg/kg. The changes of bispectral index （BIS）, MAP, HR, and serum S100β protein level at TO, T1, T2, T3, and T4 were observed. Results There were difference degrees of changes in BIS at different time points in thre groups. At T1, T2, T3, and T4, the result in group A were higher than those in group B and group C（P〈0.05）. The changes of MAP and HR were irregular, and the results of group A at T2, T3, and T4 were higher than those in the oth- er two groups（P〈O.05）. At T2, T3, and T4, the S100β protein levels were significantly lower in group A than in group B and C （P〈0.05）. Conclusion Etomidate at dose of 0.4 mg/kg applied in anesthesia can maintain the stability of haemo- dynamics, with no influeNce on S100β protein level of children, and can effectively protect the central nervous system and reduce incidence of cognitive disorder.
