摘 要:目的应用脉冲波多普勒(PW)/组织多普勒(TDI)双模式超声技术同步测量健康成年人心脏收缩期主动脉口血流峰值速度(Vmax)和速度时间积分(VTI),左室侧壁或后壁心尖水平心肌峰值速度(PV),并建立主动脉口血流参数与左室心肌峰值速度间的相关性。方法选取2012年5-6月在四川省人民医院心血管超声科行超声心动图检查的健康志愿者110例,连续同步记录3个心动周期主动脉口血流与左室侧壁或后壁心尖水平心肌运动频谱,分别测量心尖五腔心切面收缩期主动脉口血流峰值速度(Vmax-1)、速度时间积分(VTI-1)和左室侧壁心尖水平心肌峰值速度(PV-1);心尖三腔心切面Vmax-2、VTI-2与左室后壁心尖水平心肌峰值速度(PV-2)。分析并比较Vmax-1、VTI-1与PV-1以及Vmax-2、VTI-2与PV-2的相关性。结果 1)VTI-1与VTI-2差异无统计学意义(t=0.46,P>0.05);Vmax-1与Vmax-2差异有统计学意义(Z=-3.346,P=0.001)。PV-1与PV-2差异有统计学意义(Z=-6.544,P<0.001)。2)PV-1与VTI-1呈负相关(r=-0.32,P<0.01),与Vmax-1无相关性(P>0.05);PV-2与VTI-2、Vmax-2无相关性(P>0.05)。结论主动脉口血流VTI比Vmax更能体现左室侧壁心肌特征。Objective To measure the aortic peak flow velocity (Vmax) and velocity time integral (VTI) and the myocardial peak velocity (PV) in the apex of the left ventricular lateral and posterior wall by using the PW/TDI dual‐mode ultrasound technique ,and to explore the correlation between the aortic valve flow and the peak velocity of left ventricular .Methods 110 cases of healthy volunteers were selected .The motion spectrum in the aortic valve flow and the myocardial apex of left ventricular lateral and posterior walls were recorded synchronously in the three consecutive cardiac cycles ,and the echocardiography was employed during myocardium systole to measure the aortic peak flow velocity (Vmax‐1) ,the velocity time integral (VTI‐1) ,and the myocardial peak velocity (PV‐1) in the apex of the left ventricular lateral wall from the apical 5‐chaber plane ,and the aortic peak flow velocity (Vmax‐2) , the velocity time integral (VTI‐2) ,and the myocardial peak velocity (PV‐2) in the apex of the left ventricular posterior wall from the apical 3‐chaber plane . Then the parameters were compared and analyzed to explore the correlations among Vmax‐1 ,VTI‐1 and PV‐1 ,Vmax‐2 ,VTI‐2 and PV‐2 .Results First ,no statistical significance was found between VTI‐1 and VTI‐2 (t=0.46 ,P〉0.05) ,but there were statistical significances between Vmax‐1 and Vmax‐2 (Z= -3.346 ,P=0.001) and between PV‐1 and PV‐2 (Z= -6.544 ,P〈 0.001) .Second ,PV‐1 is negatively correlated with VTI‐1 (r= -0.32 ,P〈0.01) ,but it is not correlated with Vmax‐1 (P〉0.05);PV‐2 isnot correlated with VTI‐2 and Vmax‐2 respectively (P〉0.05) .Conclusion The VTI of the aortic valve flow can reflect the features of the left ventricular lateral wall better than the Vmax .
