γ-干扰素释放试验与荧光定量PCR在肺外结核诊断效果评价

摘　　要：目的研究γ-干扰素释放试验与荧光定量PCR在肺外结核的诊断效果,为提高临床确诊率提供依据。方法选取2014年10月-2015年11月医院诊治250例肺外结核患者,采用γ-干扰素释放试验与荧光定量PCR测定,将诊断结果与诊断标准进行比较,试验组γ-干扰素释放试验结果,对照组进行荧光定量PCR诊断,分析γ-干扰素释放试验与荧光定量PCR在肺外结核中的诊断效果及应用价值。结果试验组189例检测出结核分枝杆菌,阳性率75.6%,对照组86例检测出结核分枝杆菌,阳性率34.4%;试验组阳性率高于对照组,差异有统计学意义(P<0.05);试验组灵敏度为92.0%、特异性为94.0%,高于对照组的灵敏度76.0%、特异性为78.0%。结论肺外结核患者采用γ-干扰素释放试验检测阳性率和特异性较高。OBJECTIVE To study the effects ofγ-interferon release assay and quantitative PCR on the diagnosis of extrapulmonary tuberculosis,so as to improve the clinical diagnosis rate.METHODS A total of 250 cases of extrapulmonary tuberculosis patients in hospital from Oct.2014 to Nov.2015 were selected.γ-interferon release assay and fluorescent quantitative PCR assay were used,and the diagnosis results were compared with diagnostic criteria.Patients in test group were usedγ-interferon release assay,and in control group were treated by the fluorescent quantitative PCR diagnosis.The effects and application values ofγ-interferon release assay and quantitative PCR on the diagnosis of extrapulmonary tuberculosis were analyzed.RESULTS In the test group,189 cases were detected of Mycobacterium tuberculosis positive,with the positive rate of 75.6%,and 86 cases were detected of M.tuberculosis positive in control group,with the positive rate of 34.4%;the positive rate of the experimental group was nigher than of the control group,with statistical significance（P〈0.05）.The sensitivity and specificity in test group were 92.0% and 94.0%,which were higher than those of control group（76.0% and 78.0%）.CONCLUSION Usingγ-interferon release test for extrapulmonary tuberculosis patients has higher positive rate and specificity.
